Rehabilitation of the professional soccer player after LCA lesion: case report

Abstract:

The aim of this thesis is to evaluate, through a review of recent literature, the effectiveness of proprioceptive rehabilitation treatment following the intervention of ACL reconstruction but also evaluation and treatment of a clinical case in the field. To collect the necessary data, a bibliographic search was carried out using the online Pub Med databases. From the analysis of the data it was highlighted how proprioceptive re-education is important in the first phase of rehabilitation to integrate the correct movement into how much it improves the motor control, the postural stability, the sensitivity during the movement and reduces the risk of injury. Moreover the integration to the training of strength with the neuromuscular one makes the biomechanics optimal. However, this re-education alone does not seem to accelerate the return to sporting Activity and therefore must be integrated with other rehabilitation techniques. Proper intervention rehabilitation is necessary to obtain good results in the recovery of functionality of the limb after reconstruction of the ligament. In literature there are many reports on prevention programs whose common. The denominator is the correct alignment of the lower limb joints and control of the engine during movements that are considered to be at risk for integrity of the ACL, like the landing phase after a jump. However, some programs would seem more effective than others. Any how the lack of sufficient compliance with regard to is still a big problem prevention programs. Finally, it is important to remember that the etiology of ACL lesions is multifactorial. For this reason, a prevention program can prevent all risk situations is utopian.

L'obiettivo di questa tesi ? di valutare, mediante una revisione della recente letteratura, l'efficacia del trattamento riabilitativo propriocettivo in seguito all'intervento di ricostruzione del LCA ma anche valutazione e trattamento di un caso clinico sul campo. Per raccogliere i dati necessari ? stata effettuata una ricerca bibliografica usando le banche dati online Pub Med. Dall'analisi dei dati ? stato messo in evidenza come la rieducazione propriocettiva sia importante nella prima fase della riabilitazione per integrare il movimento corretto in quanto migliora il controllo motorio, la stabilit? posturale, la sensibilit? durante il movimento e riduce il rischio di infortunio. Inoltre l'integrazione all'allenamento di forza con quello neuromuscolare rende ottimale la biomeccanica. Tuttavia questa rieducazione da sola sembra non accelerare il ritorno all'attivit? sportiva e pertanto deve essere integrata ad altre tecniche riabilitative. Un corretto intervento riabilitativo ? necessario per ottenere buoni risultati nel recupero della funzionalit? dell'arto dopo ricostruzione del legamento. In letteratura ci sono molti rapporti su programmi di prevenzione il cui comune denominatore ? il corretto allineamento dell'arto inferiore articolazioni e controllo del motore durante i movimenti che sono considerati a rischio per l'integrit? dell'ACL, come la fase di atterraggio dopo un salto. Tuttavia, alcuni programmi sembrerebbero pi? efficaci di altri. In ogni caso rimane un grosso problema la mancanza di conformit? sufficiente a riguardo di programmi di prevenzione. Infine, ? importante ricordare che l'eziologia delle lesioni ACL ? multifattoriale. Per questo motivo un programma di prevenzione in grado di prevenire tutte le situazioni di rischio ? utopica.
