Economic evaluation of kidney transplant and dialysis (home/hosptal) on Czech market

Abstract:

Tato diplomová práce se zabývá ekonomickým hodnocením jednotlivých alternativ léčby selhání ledvin. Řeší identifikaci a kvantifikaci nákladů všech typů léčby a porovnává je mezi sebou. Náklady se získaly na základě rozhovorů s odborníky, řediteli, lékaři a ze sazebníku zdravotních výkonů, databáze SUKL apod. Dále v ní jde o extenzivní rešerši relevantních studií z největší světové zdravotnické databáze Pubmed, odkud se získali údaje kvality života pro výpočet analýzy nákladové užitečnosti (CUA), což bylo hlavním cílem práce. Výpočtem ICER jednotlivých metod vzniklo srovnání a graf nákladové efektivity. Výsledkem je, že vůči hemodialýze jsou dvě terapie dominantní (transplantace a peritoneální dialýza) a dvě jsou účinnější, ale dražší (domácí hemodialýza a hemodiafiltrace). Porovnáním popularity léčeb na českém a světovém trhu s výsledky analýzy vzniklo doporučení pro management zdravotnického trhu.

Abstract:

This diploma thesis deals with economic evaluation of each treatment alternative for renal failure. It solves identification and quantification of costs of all types of treatment and compares them with each other. The costs were obtained by interviews with experts, directors, doctors and from tariff of health services, SUKL database, etc. It also includes an extensive research of relevant studies from the world's largest health database Pubmed, from which quality of life data were obtained to calculate cost-effectiveness analysis ( CUA), which was the main goal of the thesis. The calculation of ICER of individual methods resulted in a comparison and a graph of cost-effectiveness. As a result, two therapies are dominant over hemodialysis (transplantation and peritoneal dialysis) and two are more effective but more expensive (home hemodialysis and hemodiafiltration). By comparing the popularity of treatments on the Czech and world markets with the results of the analysis, a recommendation for the management of the healthcare market was presented.
