Contribution of Arthroscopy in Treatment of Acute Traumatic Haemarthrosis of Knee Joint

Abstract:

Cílem disertační práce bylo zhodnotit přínos artroskopie při ošetření pacientů s akutním traumatickým hemartrosem kolenního kloubu se zaměřením na pacienty bez poranění skeletu a s normálními parametry krevní srážlivosti. Práce je rozdělena na tři části, teoretickou, experimentální a klinickou. V teoretické části jsou vyhledány literární údaje, týkající artroskopií, poranění kolene a působení krve v kolenním kloubu. Cílem experimentální části bylo ověření cévního zásobení struktur kolenního kloubu a průkaz „anatomické“ bezpečnosti instrumentace při artroskopickém ošetření akutního úrazového hemartrosu kolenního kloubu, které se týká zejména sutury menisku metodou „outside-in“. V klinické části byl hodnocen soubor 117 pacientů s poúrazovým hemartrosem kolene operovaných na I. chirurgické klinice FN U sv. Anny v Brně. Zařazeni byli pacienti po úraze kolene, u kterých bylo vypunktováno minimálně 20ml krve z poraněného kloubu. Do studie nebyli zahrnuti pacienti s poraněním skeletu patrným na prostém rtg snímku. Indikace k artroskopii byla limitována věkem 15-65 let a zařazením pacienta dle rizika anestezie nejvýše ASA II. Podle dosažených výsledků je artroskopie pro pacienty s akutním úrazovým hemartosem přínosem. Umožňuje kvalitnější ošetření některých poranění, zejména menisků, podstatně zkracuje dobu léčby a omezené mobility pacienta, čímž redukuje jejich negativní dopad zdravotní i sociální. Při dodržení vhodných limitů indikace jsou rizika tohoto postupu minimální. Z časného provedení výkonu pacient profituje jak po stránce zdravotní, tak ekonomické.

Abstract:

Aim of the dissertation work is to evaluate the benefit of arthroscopy at patients with acute traumatic knee heamathrosis, focused on patients without skeletal trauma and normal coagulation parametrs. Work is divided into three parts – theoretical, experimental and clinical. Theoretical part contains overwiev of recent publications regarding arthroscopy, knee trauma and effects of the blood in knee. Experimetal part reidenitify the vascular supply to the knee joint and defines ,, safe entry spots´´ for arthroscopic approach , specially when suturing mensicus with outside-in technique. Clinical part evaluates group of 117 pacients with traumatic knee haemarthrosis operated at I.st Surgical Dep. st. Anne´s Univ. Hospital Brno. Included were patients with knee trauma, who have had evacuated at lest of 20 ml of blood from the wounded knee. Excluded were patients with skeletal trauma visible on x-ray. Indication for the arthroscopy was limited to age group 15-65 years and preoperative ASA up to II. According to the results, arthroscopy is beneficial for patients with traumatic knee haemarthrosis. It enables better treatment of injured structures, especially meniscus. Arthroscopy decreases the time of imobilization and returns patiens earlier to their daily routine, therefore diminishes health and social impact of the injury. When given indication and limits are strictly followed, potential risk and complications are minimalized. Thus brings the asset from the early operative treatment to the injured health and also to economical well being.
