The nursing process of pregnant woman with pregestational diabetes mellitus

Abstract:

Bakalářská práce se zabývá problematikou onemocnění diabetes mellitus, který u ženy vznikl již před těhotenstvím. Tento diabetes mellitus je rozdělen do dvou skupin - I. a II. typu. Incidence tohoto onemocnění výrazně stoupá, a nejen v těhotenství přináší zdravotní rizika. Komplikace se mohou vyskytnout u matky i u plodu. Zdravotnický personál by měl být řádně informován o tomto onemocnění, jelikož je důležitá péče jak před těhotenstvím, tak během těhotenství, u porodu a po porodu. Nepochybně nesmí chybět péče o novorozence diabetické matky se sledováním potencionálních komplikací. Teoretická část se zabývá rozdíly mezi diabetes mellitus I. a II. typu. U každého typu je popsána definice onemocnění, příčiny vzniku a léčba, která je důležitá pro kompenzaci hladiny glykémie v krvi. Dále jsou zde sepsány nejčastější komplikace u matky a plodu, které se mohou objevit během samotného těhotenství nebo až za porodu a po porodu. V závěru teoretické části je popsána ošetřovatelská péče od prekoncepčního období až po období šestinedělí. V praktické části je vypracován ošetřovatelský proces u těhotné ženy s pregestačním diabetes mellitus. V úvodu je sepsána anamnéza těhotné ženy, která byla vybrána pro zpracování procesu v Městské nemocnici Ostrava - Fifejdy. Dále je popsáno fyzikální vyšetření a průběh hospitalizace. Data byla sbírána ze zdravotnické dokumentace, od zdravotnického personálu a od samotné klientky. Ošetřovatelský proces je vytvořen v doménách dle NANDA International, taxonomie II. Určené ošetřovatelské diagnózy jsou aktuální i potencionální, které musíme brát na vědomí v péči o tuto ženu.

Abstract:

The bachelor thesis looks at the issue of diabetes mellitus, which started before woman pregnancy. This diabetes mellitus is divided into two groups - I. and II. type. The incidence of this disease increases significantly, and brings health risks not only during pregnancy. Complications may occur in both the mother and the fetus. Medical staff should be properly informed about this condition as medical care is important both before and during pregnancy, delivery, and postpartum. Undoubtedly, care for diabetic mothers newborn with potential complications must not be missed. The theoretical part deals with the differences between diabetes mellitus I. and II. type. For each type is described the definition of the disease, the cause of the disease and the treatment that is important to compensate for blood glucose levels. Furthermore, the most common complications of the mother and the fetus, which may occur during pregnancy or during and after childbirth, are listed. At the end of the theoretical part is described nursing care from pre-conception period to postpartum period. In the practical part is elaborated nursing process for pregnant woman with pre-gestational diabetes mellitus. In the introduction is captured history of a pregnant woman who was selected to capture the process at the Ostrava - Fifejdy Municipal Hospital. Furthermore, the physical examination and the course of hospitalization are described. The data were collected from medical records, from medical staff and from the client. Nursing process is created in NANDA International, Taxonomy II domains. Designated nursing diagnoses are both current and potential, which we need to take into account in the care of this woman.
