Physiotherapy in low back pain

Abstract:

Úvod: Bolest dolních zad patří mezi nejčastější onemocnění, jsou důvodem časté pracovní neschopnosti, bolestmi pocházejícími z páteře trpí během života 65-80 % veškeré populace. Cíl: Cílem práce je lépe porozumět problematice bolesti dolních zad, přispět k jejímu řešení přehledem fyzioterapeutických technik a sestavením individuálního fyzioterapeutického plánu pro odstranění bolesti dolních zad u pacientky. Metodika: Byla zpracována kazuistika čtyřicetileté ženy v ambulantní péči, která trpěla déle než rok bolestmi v lumbosakrální oblasti. Na základě kineziologického rozboru byl vytvořen krátkodobý fyzioterapeutický plán. Byla realizována dvanáctitýdenní fyzioterapeutická intervence v 10 návštěvách. Při terapii byly použity metody diagnostiky a terapie funkčních poruch, dynamická neuromuskulární stabilizace, proprioceptivní neuromuskulární facilitace, senzomotorická stimulace, spirální stabilizace páteře, kinesiotaping a Computer Kinesiology. Byla provedena korekce špatných pohybových stereotypů, doporučeny podpůrné pomůcky pro správný sed a správná ergonomie pracovního místa. Výsledky: Výstupní kineziologický rozbor prokázal úspěšnost terapie, osvědčilo se aktivní cvičení na neurofyziologickém podkladě, došlo ke zlepšení posturálních funkcí, posílení hlubokých stabilizátorů páteře a břišních svalů, redukovalo se subjektivní vnímání bolesti v lumbosakrální oblasti. Byla pozorována závislost zlepšení na psychickém stavu, což dokazuje komplexnost problematiky. Závěr: Fyzioterapeutická intervence je prospěšná pro léčbu bolesti dolních zad.

Abstract:

Introduction: Low back pain is one of the most common disease and it is frequent cause of absenteeism. About 65-80% of the population sometimes suffers by vertebral pain during life. Objective: The aim is to better understand the problem of low back pain and contribute to its solution by overview of physiotherapy techniques and by assembling of individual physiotherapy plan to eliminate low back pain in a patient. Methodology: A case report of 40years old woman in outpatient care was made. Patient suffered by pain in the lumbosacral region more than a year. Short-term physiotherapy treatment plan was made based on the physiotherapy assessment report. Ten physiotherapy interventions in twelve weeks followed. Methods of diagnosis and treatment of functional disorders, dynamic neuromuscular stabilization, proprioceptive neuromuscular facilitation and sensorimotor stimulation, spiral stabilization of the spine, kinesiotaping and Computer Kinesiology were used during therapy. Incorrect motion stereotypes were corrected, supportive aids for correct sitting posture were recommended and proper ergonomics of the workplace was set. Results: Good results of the therapy were confirmed by final physiotherapy report. The results proved that used active exercise on the neurophysiological basis was correct. Major improvements were: positive progress of postural functions, strengthening of deep stabilizers of the spine and abdominal muscles, reduction the subjective perception of pain in the lumbosacral region. A positive correlation between the above mentioned improvements and improvement of mental state of patient was observed. Conclusion: Physical intervention is beneficial for the treatment of low back pain.
