The influence of idiopathic macular hole surgery to electrofysiological functions of the retina

Abstract:

Mnoho současných klinických prací dokazuje příznivý vliv pars plana vitrektomie (PPV) s peelingem membrana limitans interna (MLI) a plynnou tamponádou pro idiopatickou makulární díru (IMD) na morfologii makuly. Dokazují zmenšení či vymizení centrálního skotomu a metamorfopsií a především také zlepšení nejlépe korigované zrakové ostrosti (NKZO) do dálky i do blízka. Zhodnocení objektivního funkčního stavu sítnice je ale nadále diskutovanou otázkou. Práce se zabývá srovnáním elektrické funkce sítnice před a po operaci IMD s peelingem MLI a plynnou tamponádou. Do souboru bylo zařazeno 42 očí s IMD, u 3 pacientů jsme zařadili obě oči. Soubor tvořilo celkem 39 pacientů (28 žen, 11 mužů). Průměrný věk byl 66±7 let, medián 66 let. Soubor tvořili pacienti s transparentními optickými médii. U žádného z těchto pacientů nebyla nalezena přítomnost jiné makulární patologie než IMD. Nikdo neprodělal předchozí operaci sítnice. U všech pacientů byla v letech 2007-2009 na Oční klinice FN Brno provedena PPV s peelingem MLI a plynnou tamponádou pro IMD, operace provedli 2 chirurgové. Celý soubor byl, vzhledem ke statistickému zhodnocení a srovnání hodnot, dále rozdělen do 2 souborů. Jednalo se o soubor bez peroperačních či pooperačních komplikací ovlivňujících elektroretinogram (ERG), kam bylo zařazeno celkem 30 očí. Soubor s komplikacemi ovlivňujícími ERG vyšetření čítal 12 očí. Soubor očí bez komplikací byl vyšetřen 1 den před operací, 1, 3, 6 a 12 měsíců po operaci IMD. Soubor s komplikacemi byl vyšetřen 1 den před operací a 1 rok po operaci IMD. Při každé kontrole byla provedena následující vyšetření: Amslerův test, nejlépe korigovaná zraková ostrost (NKZO) do blízka (Jaegerovy tabulky), NKZO do dálky (ETDRS optotypy), vyšetření nitroočního tlaku bezkontaktním tonometrem (NIDEK NT-2000), vyšetření předního segmentu na štěrbinové lampě, vyšetření očního pozadí biomikroskopicky i nepřímou oftalmoskopií, vyšetření fotopického (F), pattern (P) i multifokálního (Mf) ERG (Retiscan dle metodiky ISCEV) a OCT vyšetření (Stratus OCT). Dle potřeby bylo provedeno i UZV vyšetření (Ultrascan Alcon). ERG vyšetření jsme provedli dle standardů klinické elektroretinografie stanovených ISCEV. Použili jsme nitkovité kontaktní DTL elektrody. Při vyšetření multifokálního ERG jsme sítnici stimulovali pomocí 61 binárních hexagonů. Ke statistickému zpracování hodnot NKZO do dálky (log MAR) a hodnot ERG komponent mezi soubory v čase ve vztahu k operaci IMD jsme použili Mann-Whitney U-Test. Byly srovnány jednotlivé parametry F, P a Mf ERG. U Mf ERG jsme hodnotili počítačem extrahované hodnoty komponent zvlášť v centrálním (4º kolem oblasti fixace) a zvlášť v paracentrálním prstenci (5-10º kolem oblasti fixace). U souboru bez komplikací došlo 12 měsíců po operaci IMD ke statisticky signifikantnímu zlepšení NKZO do dálky o 0,35 log MAR (3-4 řádky na ETDRS optotypech). U souboru s komplikacemi nedošlo dle statistiky 12 měsíců po operaci ani ke zlepšení ani ke zhoršení NKZO do dálky. U souboru bez komplikací, při srovnání ERG parametrů, byly statisticky a klinicky významné následně uvedené výsledky. U F ERG byla statisticky signifikantně prodloužena latence b u souborů 1, 3, 6 a 12 měsíců po operaci oproti souboru 1 den před operací. U P ERG byla statisticky signifikantně prodloužena latence P50 u souborů 1, 6 a 12 měsíců po operaci oproti souboru 1 den před operací. U Mf ERG bylo nalezeno statisticky signifikantní zvýšení amplitudy P1 v centrálním prstenci u souboru 1, 3, 6 a 12 měsíců po operaci oproti souboru 1 den před operací. V paracentrálním prstenci bylo nalezeno statisticky signifikantní prodloužení latence P1 u souboru 12 měsíců po operaci oproti souboru 1 den před operac

Abstract:

í. U souboru pacientů s komplikacemi došlo pooperačně a dále i 1 rok po operaci dle ERG ke statisticky signifikantnímu snížení amplitudy b u F ERG, amplitudy N95 u P ERG a amplitudy P1 v paracentrálním prstenci u Mf ERG. U Mf ERG nebyl v centrálním prstenci nalezen statisticky významný rozdíl amplitudy P1 mezi souborem před operací a 1 rok po operaci. Na základě výsledků ERG vyšetření našeho souboru bez komplikací došlo jednoznačně ke zlepšení funkce fovey již těsně pooperačně a dále ještě více i 1 rok po operaci IMD. V ostatních částech sítnice, v oblasti parafovey a komplexně na celé sítnice, nedošlo v 1 ročním pooperačním sledování ke snížení ani zvýšení počtu funkčních nervových buněk, ale došlo k významnému prodloužení vedení vnějšími vrstvami sítnice. V oblasti parafovey a i komplexně na celé sítnici došlo spíše ke zhoršení funkce vnějších vrstev sítnice. Oblast parafovey se ve velkém rozsahu shoduje s oblastí peelingu MLI. I když mnohé studie prokázaly významný přínos peelingu MLI na zvýšení počtu uzavřených děr a snížení rizika znovuotevření díry, tak i dle výsledků naší práce je diskuze ohledně indikace peelingu MLI u nižších stádií IMD jistě na místě. Vývoj elektrofyziologické funkce sítnice po nekomplikované operaci IMD s peelingem MLI a plynnou tamponádou budeme nadále sledovat. U souboru s komplikacemi nedošlo ve fovee dle ERG 1 rok po operaci ani ke zlepšení ani ke zhoršení její funkce. Funkce sítnice komplexně, funkce makuly i parafoveolární oblasti byla ale zhoršena ve smyslu snížení počtu nervových buněk sítnice ve vnějších i vnitřních vrstvách. Je zřejmé, že v průběhu PPV je třeba klást důraz na preciznost mikrochirurgie, aby se zmírnilo riziko peroperačních i pooperačních komplikací. Především důsledná kontrola periferní sítnice při PPV, důsledné peroperační ošetření, umožní předejít pooperačnímu odchlípení sítnice, které nenávratně postihuje její funkce.

Abstract:

Many contemporary clinical papers establish positive influence of the pars plana vitrectomy (PPV) with the internal limiting membrane (ILM) peeling and gas tamponade in macular hole to the macular morphology. They prove diminishing or disappearing of the central scotoma and metamorphopsia and especially also improvement of the best corrected visual acuity (BCVA) for far and near. The evaluation of the objective functional condition of the retina is still a discussed question. This paper concerns with the comparison of the electric functions of the retina before and after the idiopathic macular hole (IMH) surgery with the ILM peeling and the gas tamponade. In the group were included 42 eyes with IMH, in 3 patients we included both eyes. The group consisted of 39 patients (28 women, 11 men). The average age was 66 ± 7 years, median 66 years. The group consisted of patients with a transparent optical media. In none of these patients was found other macular pathology than IMH. Nobody underwent other retinal surgery. All patients were treated in the Eye Clinic, Faculty Hospital of Brno between years 2007-2009, where was performed PPV with ILM peeling and gas tamponade for IMH, two surgeons performed the operations. The entire group was divided into 2 files due to statistical evaluation and comparison of the values. First is the file without intraoperative or postoperative complications affecting the electroretinography (ERG), which included 30 eyes. In the group with complications affecting ERG were 12 eyes. The group of eyes without complications was examinated 1 day before surgery, 1, 3, 6 and 12 months after IMH surgery. The group of eyes with complications was examinated 1 day before surgery and 1 year after IMH surgery. During each control, the following examinations were performed: the Amsler grid examination, the best corrected visual acuity (BCVA) for far (ETDRS chart) and near (Jaeger optotypes), intraocular pressure measurement (non contact tonometer NIDEK NT-2000), examination of the anterior segment on the slit lamp, examination of the posterior segment biomicroscopically and by means of indirect ophthalmoscopy, examination of the photopic (Ph), pattern (P), and multifocal Mf) ERG (Retiscan, according to the ISCEV methodology), and OCT examination (Stratus OCT). If necessary, the ultrasound examination (Ultrascan Alcon) was performed as well. ERG testing was performed according to standards of clinical electroretinography of ISCEV. We used DTL contact electrodes. In the Mf ERG examination, we stimulated the retina with 61 binary hexagons. For the statistical processing of values BCVA for far (log MAR) and values of ERG components between groups in the relation to the time of IMH operation, we used the Mann-Whitney U-test. We compared parameters of the F, P and Mf ERG. In the Mf ERG, we assessed values of computer- extracted components separately in the central (4 ° around the fixation) and particular in the paracentral ring (5-10 ° around the fixation). In the group without the complication occurred 12 months after IMH surgery statistically significant improvement of BCVA for far by 0.35 log MAR (3-4 lines on ETDRS optotypes. In the group with complications occured according to statistics 12 months after surgery no improvement or deterioration BCVA for far. In the group without complications were statistically and clinically significant results of the comparison of followed ERG parameters. In the Ph ERG, there was statistically significant prolonged latency b in data files 1, 3, 6 and 12 months after surgery comparing to the data file 1 day before surgery. In the P ERG, there was statistically significant prolonged latency P50 in data file 1, 6 and 12 months after surgery comparing to the data file 1 day before surgery. urgery. In paracentral ring, there was found a statistically significant prolongation of latency P1 in the data file 1 year after surgery comparing in the data file 1 day before surgery. In the group of patients with complications, there were occurred in postoperative ERG results and 12 months after surgery statistically significant reduction of amplitude b in the Ph ERG, amplitude N95 in the P ERG and amplitude P1 in the paracentral ring in the Mf ERG. In the Mf ERG, there was no statistically significant difference in P1 amplitude between the data file before and 1 year after surgery in the central ring. In the group without complications, there was statistically significant improvement of the retinal electric function in the fovea postoperatively and more 1 year after the surgery. It confirmed the positive functional effect of the surgery to the fovea. In the fovea, there was occured the increase of the number of functional nerve cells in outer layers of the retina. On the other hand, in the parafoveolar region, as well as in the whole retina, postoperatively and 1 year after the surgery, there was statistically significant decrease of the function of the retina, meaning the time prolongation of the conduction in outer layers of the retina. According also to our results, the peeling of the ILM in the IMH surgery remains, despite its unquestionable contribution, still a controversial technique. During the 1 year lasting follow-up period, there was occured the functional improvement in the fovea, but the functional decrease in the parafoveolar region, which correlates in the large extent with area of the ILM peeling. The discussion about the ILM peeling indication in the earlier stages is adequate. We will further follow-up the development of the retinal electric function after the IMH surgery with ILM peeling and gas tamponade. In the group with complications, there was occurred no improvement or deterioration of the function in the fovea 1 year after surgery. Complex function of the retina, the macula and the parafoveolar area, there were decreased the number of retinal nerve cells in the outer and inner layers. It is clear, that during PPV is necessary to emphasize the precision of microsurgery to reduce the risk of perioperative and postoperative complications. In particular, a thorough inspection of the peripheral retina during PPV, intraoperative rigorous treatment can prevent postoperative retinal detachment, which irreversibly affects its function.
