Detection of deletions of SHOX gene

Abstract:

Moje bakalářská práce pojednává o 20. pacientech s podezřením na mikrodeleci lokalizované v genu SHOX a jeho regulačních oblastí, jehož mikrodelece či delece je příčinou malého vzrůstu. To je hlavní klinický příznak, podle kterého bylo vybráno 21 probandů. Tito pacienti byli v minulosti vyšetřeni na tuto deleci metodou fluorescenční in situ hybridizace a byli negativní. V mé práci je haploinsuficience genu SHOX zjišťována pomocí metody MLPA. Jde tudíž o srovnání dvou metod, zda novější metoda je schopna odhalit mikrodelece, které FISH neodhalila. Základní kritéria, podle kterých byli pacienti vybráni, je malý vzrůst a negativní výsledek analýzy delece SHOX metodou FISH. S větší četností se také u pacientů objevil opožděný kostní věk, zkrácené segmenty dlouhých kostí, vysoké patro a disproporční postava. Typický příznak delece v oblasti genu SHOX, Madelungova deformita, byla objevena u 4 pacientů. K detekci haploinsuficience SHOX genu byla použita metoda MLPA, při níž byl použit kit SALSA MLPA P018-61 a následná fragmentační analýza byla provedena na genetickém analyzátoru ABI3130. V souboru pacientů vyšetřených touto metodou bylo nalezeno 5 delecí v oblasti genu SHOX a jeho regulačních oblastí. Jednalo se o rodinný výskyt této delece, kdy pozitivní pacienti měli normální mužský či ženský karyotyp.

Abstract:

My bachelor thesis deals with the 20 patients suspected on the basis of micro deletion located in the SHOX gene and its regulatory sections where the deletion or micro deletion takes place and causes the short stature. 21 patients have been selected due to this main clinical symptom. These patients have been previously examined for such a deletion by fluorescence in situ hybridization but results were negative after all. My research is focused on haploinsuficiency SHOX gene which is determined by using the MLPA method. The comparison of two methods have been applied to find out whether newer method is able to reveal micro deletions in order to FISH method. Patients have been selected on the basis of short stature and negative SHOX results based on FISH method considering just a basic criteria for such a selection. Once the amount of examined patients have been increased, the symptoms occurred in the matters of delayed bone age, shortened segments of long bones, high floor and disproportionate figure. Typical symptoms of deletions in the SHOX gene, called as Madelung deformity, have been discovered within 4 examined patients. MLPA method has been used to detect haploinsuficiency gene SHOX as well as the SALSA MLPA P018-61 kit has been applied. Following fragment analysis has been executed on genetic analyser ABI3130. 5 deletions with relation to SHOX gene and its regulatory sections based on this MLPA method have been found in the sample of examined patients. Such a deletion has been determined as related occurrence where the positive patients had normal men or women karyotype.
