Factors influencing the illness perception in adolescents with type I diabetes

Anotácia:

Cílem této práce bylo zjistit, které faktory ovlivňují pojetí nemoci a péči o sebe u adolescentů s cukrovkou I. typu. Byl testován vliv kvality vztahu s matkou, self-efficacy adolescenta a úzkostnosti matky na péči o sebe a pojetí nemoci adolescenta. Dále bylo zjišťováno, jak dobře tyto proměnné společně s věkem a pohlavím adolescenta dokáží vysvětlit jeho pojetí nemoci a péči o sebe. Výzkumu se účastnilo celkem 30 dvojic matka-dítě, věk adolescentů byl 14-20 let. Sledované konstrukty byly měřeny pěti standardizovanými dotazníky. Korelační analýzou se podařilo potvrdit, že kvalita vztahu s matkou, self-efficacy adolescenta i úzkostnost matky velmi úzce souvisí s péčí o sebe. Souvislost kvality vztahu s matkou a self-efficacy adolescenta s jeho pojetím nemoci se také podařilo prokázat, vztah mezi úzkostností matky a pojetím nemoci adolescenta však nikoliv. V regresním modelu se jako jediný významný prediktor péče o sebe i pojetí nemoci ukázalo self-efficacy, které zprostředkovávalo vliv kvality vztahu s matkou, úzkostnosti matky a věku adolescenta na jeho péči o sebe, a vliv kvality vztahu s matkou na jeho pojetí nemoci. Pohlaví adolescenta nemělo významný vliv na jeho péči o sebe, stejně tak jako věk adolescenta na jeho pojetí nemoci. Bylo diskutováno možné využití těchto poznatků v psychologické péči o adolescenty s cukrovkou, u kterých by se mohlo vhodnou intervencí zaměřenou především na zvýšení self-efficacy zlepšit jejich pojetí nemoci i péče o sebe.

Abstract:

The aim of this thesis was to find out which factors influence illness perception and self-care in adolescents with type I diabetes. Influence of quality of relationship with mother, adolescents´ self-efficacy and maternal anxiety on illness perception and self-care was tested. Furthermore, it was elaborated how these variables together with age and gender of the adolescents can explain their illness perception and self-care. Thirty mother-child pairs participated in the research; age of the adolescents was 14-20 years. The constructs were measured by five standardized questionnaires. Correlation analysis confirmed that quality of the relationship with mother, self-efficacy and maternal anxiety are closely related to self-care. There was also found relationship between quality of the relationship with mother, adolescent´s self-efficacy and his illness perception. However, there was no relationship between maternal anxiety and adolescent´s illness perception. Self-efficacy was the only significant predictor of self-care and illness perception in a model of linear regression. This construct mediated influence of quality of the relationship with mother, maternal anxiety and adolescents´ age on their self-care and the influence of quality of the relationship with mother on their illness perception. Adolescents´ gender had no significant effect on their self-care. Further, the effect of adolescents´ age on their illness perception was also insignificant. The possible application of this findings in psychological care for adolescents with type I diabetes was discussed. An appropriate intervention focused primarily on increase of self-efficacy could improve illness perception and self-care.
