Abstract:

Bakalářská práce se zabývá specifiky ošetřovatelské péče u pacientů po artroskopii ramene s různými typy používaných fixací horní končetiny. Teoreticky popisuje základní vyšetření ramenního pletence, obecné principy artroskopie, ošetřovatelskou a rehabilitační péči a také fixaci horní končetiny související s artroskopií ramene. V průzkumné části jsou zpracována retrospektivní demografická data a dané údaje vybraných pacientů. Dále jsou uvedené tři kazuistiky pacientů podle typů používaných fixací horní končetiny. Praktickým výstupem této práce je vytvořená ošetřovatelská dokumentace pro záznam péče, která se týká pacientů po artroskopii ramene s respektováním typu fixace horní končetiny. Retrospektivním průzkumem bylo zjištěno, že za rok 2019 bylo k artroskopii indikováno 72 pacientů. Z tohoto počtu bylo více pacientů mužů. Naopak z celkového počtu 28 pacientů v roce 2020 bylo artroskopicky operováno více žen. Těmto pacientům byl pooperačně nasazen šátkový závěs, Desaultova nebo abdukční ortéza. Zjištěná data ukázala, že ošetřovatelská péče o ně byla ovlivněna typem jejich fixace. U pacientů s Desaultovou ortézou byla pooperační nervová blokáda využita nejvíce a zároveň byla u nich také nejúčinnější. Redonova drenáž byla zavedena nejčastěji u pacientů s abdukční ortézou. S hygienickou péčí bylo nejvíce potřeba dopomoci pacientům s Desaultovou a abdukční ortézou. Pohybová aktivita a stravování bylo nejvíce deficitní u pacientů s Desaultovou ortézou. S oblékáním bylo potřeba nejvíce dopomoci pacientům se šátkovým závěsem.

Abstract:

The bachelor thesis deals with the specifics of the nursing care in patients after shoulder arthroscopy with different types of upper limb fixation. It describes theoretically a basic examination of the shoulder girdle, general principles of arthroscopy, nursing and rehabilitation care as well as upper limb fixation related to shoulder arthroscopy. The research part processes retrospective demographic data and the given data from selected patients. Furthermore, three patients´ casuistries are presented according to the used types of upper limb fixation. The practical outcome of this thesis is a created nursing documentation for a care record, which concerns patients after shoulder arthroscopy with respect for the type of upper limb fixation. Through a retrospective research it was revealed that in 2019, 72 patients were indicated for arthroscopy. There were more male patients from this amount. On the other hand, from the total number of 28 patients in 2020, more females had an arthroscopy surgery. These patients were given an elevation sling, Desault´s bandage or abduction bandage after the surgery. The obtained data revealed that nursing care of these was influenced by a type of their fixation. The post operative nerve block was used in patients with a Desault´s bandage and simultaneously it was also the most efficient here. Redon drainage was inserted in patients with an abduction bandage most frequently. The patients with Desaults´s and abduction bandages needed help with hygienic care most frequently. Physical activity and eating were most deficient in patients with a Desaults´ bandage. Patients with an elevation sling needed help with dressing most frequently.
