Effect of targeted physiotherapy on static and dynamic parameters of spirometry in performance cyclists

Abstract:

Cílem této diplomové práce bylo zjistit, zda má cílená fyzioterapie vliv na statické a dynamické parametry spirometrie u výkonnostních cyklistů. Výzkumu se zúčastnilo 20 výkonnostních cyklistů ve věku 17 - 48 let (průměrný věk 30,15 ? 10,2 let). Podmínkou pro přijetí byl počet ročně ujetých kilometrů v rozmezí 10 - 15 tisíc. Po vstupním vyšetření byli probandi rozdělení po deseti lidech do skupiny experimentální (26,9 ? 8,5 let ; 180,6 ? 5,4 cm ; 74,8 ? 10,9 kg ) a kontrolní (33,4 ? 11,2 let; 179,2 ? 7 cm; 72,7 ? 8,7 kg). Rozdělení do skupin proběhlo podle indexu sagitální pohyblivosti hrudní páteře a podle pravidelnosti kompenzačního cvičení probanda zodpovězené v anamnestické anketě. Cvičební jednotka se zaměřovala na zlepšení poddajnosti hrudního koše pomocí manuální terapie následovanou cílenou kinezioterapií inspirující se vývojovou kineziologií. Intervence probíhala od 1.12.2023 do 29.2.2024. Délka jedné intervence činila 45 minut a každý proband absolvoval celkem 10 terapií s časovým odstupem jednoho týdne. V rámci intervence byly doporučeny jednotlivé cviky, které probandi v rámci autoterapie cvičili 2x týdně a pravidelnost cvičení si zaznamenávali do tréninkových deníků. Ověření úspěšnosti proběhlo srovnáním vybraných vstupních a výstupních ventilačních parametrů plic měřených pomocí spirometru SPIRODOC. Z antropometrického měření bylo ověření úspěšnosti doplněno srovnáním indexu sagitální pohyblivosti hrudní páteře a funkční kapacitou hrudního koše. Po provedení intervencí došlo u obou skupin ke statisticky významnému zlepšení vybraných ventilačních parametrů (VC, FEV1/FVC). V rámci antropometrického měření došlo u experimentální skupiny ke statisticky významnému zlepšení indexu sagitální pohyblivosti hrudní páteře, zejména u Ottova reklinačního indexu. Funkční kapacita hrudního koše byla statisticky významně zlepšena u obou skupin. Tato práce poukazuje na to, že cílená fyzioterapie má své zastoupení u výkonnostní cyklistiky při zlepšování mobility hrudního koše, což může následně mít pozitivní vliv na statické a dynamické parametry spirometrie.

Abstract:

The aim of this master's thesis was to determine whether targeted physiotherapy has an impact on static and dynamic spirometric parameters with performance cyclists. Twenty performance cyclists aged 17 to 48 years (average age 30.15 ? 10.2 years) participated in the study. The inclusion criterion was the number of kilometers cycled annually, ranging from 10,000 to 15,000. After the initial examination, the subjects were divided into two groups of ten: an experimental group (26.9 ? 8.5 years; 180.6 ? 5.4 cm; 74.8 ? 10.9 kg) and a control group (33.4 ? 11.2 years; 179.2 ? 7 cm; 72.7 ? 8.7 kg). Group allocation was based on the sagittal mobility index of the thoracic spine and the regularity of compensatory exercises reported by the subjects in the anamnestic questionnaire. The exercise unit aimed to improve thoracic cage compliance through manual therapy followed by targeted kinesiotherapy inspired by developmental kinesiology. The intervention took place from December 1, 2023, to February 29, 2024. Each intervention lasted 45 minutes, and each subject completed a total of 10 therapies spaced one week apart. During the intervention, specific exercises were recommended which the subjects practiced twice a week as a self-therapy and recorded the exercise frequency in training diaries. The effectiveness was verified by comparing selected initial and final ventilatory parameters of the lungs measured by the SPIRODOC spirometer. Anthropometric measurements were complemented by comparing the sagittal mobility index of the thoracic spine and the functional capacity of the thoracic cage. After the interventions, both groups showed statistically significant improvement in selected ventilatory parameters (VC, FEV1/FVC). In terms of anthropometric measurements, the experimental group showed statistically significant improvement in the sagittal mobility index of the thoracic spine, particularly in Ott's reclination index. The functional capacity of the thoracic cage was significantly improved in both groups.This study highlights that targeted physiotherapy has its place in performance cycling for improving of the thoracic cage mobility which may subsequently have a positive impact on static and dynamic spirometric parameters.
