The Use of Vacuum Therapy on the Healing of Chronic Wounds

Abstract:

ABSTRAKT Cíl: Cílem bakalářské práce je zjistit, zda podtlaková terapie urychluje hojení chronických ran. Zjistit, zda je využití podtlaku užitečnější, nežli vlhká terapie. Zjistit, zda pacienti lépe snášejí bolest při léčbě s využitím podtlaku. Zjistit, zda je podtlaková terapie finančně výhodnější. Byla stanovena klinická otázka v PICO formátu: P (pacient) ? pacient s ránou či dehiscencí, I (intervence) ? podtlaková terapie, C (srovnání) ? vlhké hojení, O (výsledek) ? urychlení hojení. Metodika: Zařazené studie byly vyhledány v elektronických databázích Bibliographia Medica Čechoslovaca (BMČ), Google, Google Scholar, MEDVIK, PubMed, Science Direct, Springer link US a v časopisech a knihách uvedených v bibliografických zdrojích publikovaných od roku 2005 ? 2015 s pomocí klíčových slov podtlaková terapie, podtlak, hojení ran, čas hojení, debridement. Vyhledávání bylo zaměřeno na analytické a deskriptivní studie, kazuistiky, klinické případové studie, kohortové studie, průřezové studie, randomizované kontrolované studie (RCT). Výsledky: Zařazující kritéria splnilo 23 studií, které se zabývají podtlakovou terapií v hojení ran, urychlením hojení pomocí podtlaku, nižšími náklady na léčbu formou přiložení podtlaku a různá využití negativního tlaku na ránu. Závěr: Důkazy studií prokázaly zrychlení hojení pomocí podtlakové terapie, prokázaly rychlejší nárůst granulační tkáně a zmenšení velikosti rány. Podtlaková terapie šetří náklady na léčbu s porovnáním se standardní léčbou. Patří zde i větší spokojenost pacientů, personálu a dochází ke zlepšení kvality života u pacientů s chronickou ránou. Abstract:

ABSTRACT Objective: The aim of this work is to determine whether vacuum therapy accelerates healing of chronic wounds. Determine whether the use of vacuum is useful than moist therapy. Determine whether patients better tolerate pain during treatment using a vacuum. Determine whether vacuum therapy is cost-effective. Clinical question in PICO format was set: P (patient) - patients with wound or dehiscence, I (intervention) - vacuum therapy, C (comparison) - moist wound, O (result) - accelerate healing. Methods: Included studies were searched in electronic databases Bibliographia Medica Čechoslovaca (BMC), Google, Google Scholar, MEDVIK, PubMed, Science Direct, Springer Link US and in magazines and books listed in the bibliography sources published since 2005 to 2015 using the keywords vacuum therapy, depression, wound healing, healing time, debridement. The search was focused on analytical and descriptive studies, case reports, clinical case studies, cohort studies, cross-sectional study, a randomized controlled trial (RCT). Results: Classifying criteria fulfill 23 studies dealing with vacuum therapy on wound healing, acceleration of healing using negative pressure, lower treatment costs through applying the vacuum and various uses negative pressure to the wound. Conclusion: Evidence of studies have shown accelerating the healing process using vacuum therapy showed faster growth of granulation tissue and reducing the size of the wound. Vacuum therapy saving treatment costs in comparison with standard therapy. This includes even greater satisfaction for patients, staff and improves the quality of life for patients with chronic wounds.
