Screening, diagnosis and treatment of retinopathy of prematurity

Abstract:

Úvod. Retinopatie nedonošených (ROP) je vazoproliferativní onemocnění postihující zejména předčasně narozené děti s nízkou porodní hmotností. Hlavními rizikovými faktory jsou stupeň nezralosti a nízká porodní hmotnost, predikující incidenci, průběh a prognózu ROP, nicméně na vzniku se může podílet řada dalších faktorů. Cíle. Prvním z cílů této práce byla identifikace rizikových faktorů a prozkoumaní jejích vztahů k vzniku a vývoji ROP. Dalším cílem bylo zhodnocení efektivity systému WinROP, vyvinutého pro včasnou detekci závažných forem ROP potenciálně vyžadujících léčbu. Posledním cílem bylo popsat na našem pracovišti zavedenou novou modifikovanou techniku léčby ROP pod vizuální kontrolou zobrazovacího systému RetCam 3 – kamerou asistovanou laserovou fotokoagulaci a kryoterapii sítnice (metoda CALCR). Metodika. Retrospektivně byla analyzována zdravotní dokumentace 445 dětí narozených před 32. gestačním týdnem ve Fakultní nemocnici v Ostravě v době od 1. 11. 2011 do 31. 10. 2014, které byly sledovány v rámci screeningu ROP, včetně dat získaných ze systému WinROP. U 19 dětí léčených metodou CALCR byl sledován výsledek terapie včetně perioperačních a postoperačních komplikací. Výsledky. ROP byla diagnostikována u 31 % dětí v souboru, léčbu vyžadovalo 14 % z nich. Byl potvrzen vliv následujících rizikových faktorů na výskyt a závažnost ROP: gestační věk při narození, porodní hmotnost, APGAR skóre v 1. minutě po narození, umělé plicní ventilace, oxygenoterapie a terapie min. 30% FiO2 o délce trvání minimálně 120 hodin, podání transfuze erytrocytů, sepse nebo infekce perinatálně, antibiotické terapie, syndromu respirační tísně, bronchopulmonální dysplazie, podání surfaktantu, vasopresorů a přítomnosti nekrotizující enterokolitidy, perzistujícího arteriózního duktu nebo intraventrikulárního krvácení 3. - 4. stupně. Naopak nebyl nalezen vztah mezi výskytem ROP a délkou fototerapie nebo podáním plné kúry kortikosteroidů matce. Systém WinROP prokázal 100% senzitivitu i prediktivní hodnotu negativního testu u všech předčasně narozených dětí s ROP typu 1 či 2, které vyžadují nebo mohou vyžadovat léčbu. Pozitivní prediktivní hodnota i specificita však byly relativně nízké. Pro detekci méně závažných forem retinopatie není systém příliš spolehlivý. U dětí léčených metodou CALCR byla terapie úspěšná bez závažnějších komplikací. Závěr. Výše uvedené rizikové faktory mohou přispět k lepšímu rozpoznávání rizikových dětí. WinROP systém se v současné podobě jeví jako užitečný doplňkový nástroj pro screening ROP v běžné klinické praxi. Přestože specificita nebyla příliš vysoká a systém produkoval značné množství falešně pozitivních výsledků, systém je výborně použitelný jako prostředek pro snížení počtu nutných oftalmologických vyšetření u novorozenců, které systém neoznačí jako rizikové. CALCR je bezpečná a efektivní metoda léčby ROP u dětí se změnami v zóně II sítnice anteriorně.

Abstract:

Background. Retinopathy of prematurity (ROP) is a vasoproliferative disease affecting predominantly prematurely born, low birth-weight infants. These factors, i.e., the degree of prematurity and low birth weight, are considered the principal risk factors predicting the incidence, course and outlook of the disease. Multiple other factors may however also play a role. Purpose. The first aim of this Thesis was to identify various risk factors and to investigate their relation to the ROP development. The second aim was to evaluate the effectiveness of WinROP system – an online system developed for timely detection of serious forms of the disease that may require treatment. The last aim was to describe a novel modified technique used in our department, utilizing a RetCam 3 imaging system for ROP treatment – Camera-Assisted Laser photocoagulation and Cryotherapy of the Retina (CALCR) Methods. A retrospective analysis of documentation of 445 children born before week 32 of gestation in the University Hospital Ostrava from 1. 11. 2011 to 31. 10. 2014 included into our ROP screening program was performed. Data acquired from the ROP screening system were also included in the analysis. The effect of the therapy using CALCR method in 19 patients requiring treatment was also evaluated, including the perioperative and postoperative complications. Results. ROP was diagnosed in 31 % of patients in the group, 14% of those needed treatment. The following risk factors were found to have a significant effect on the incidence and gravity of ROP: gestational age at birth, birth-weight, APGAR score one minute after birth, mechanical ventilation, oxygen therapy and therapy using FiO2 ≥ 30% for a minimum of 120 hours, transfusion of leukoreduced red blood cells, perinatal sepsis or infection, antibiotic therapy, respiratory distress syndrome, vasopressor or surfactant administration, and presence of necrotizing enterocolitis or intraventricular hemorrhage of grade III-IV. Contrary, no relationship was found between the ROP incidence and the duration of phototherapy or antenatal administration of corticosteroids to the mother. The WinROP system demonstrated a 100% sensitivity and 100% negative predictive value in detecting prematures with ROP type 1 or 2 who require treatment or are likely to require it. The positive predictive value and specificity were however low. The system did not provide reliable results when used for detection of less serious forms of ROP. Conclusions. Revealing the above listed risk factors can contribute to a better detection of children in high risk of ROP development. At present, WinROP seems to be a very useful auxiliary tool for ROP screening in the everyday clinical practice. Although the specificity was not particularly high and the system produced a relatively high number of false positive results, it can be successfully used as a tool for reducing the number of necessary ophthalmological investigations in newborns who are not denoted as „high-risk“ by the system. CALCR is a safe and effective method for treatment of ROP in children with changes in the anterior part of the zone II.
