Abstract:

Pacienti se závažným duševním onemocněním (severe mental illness, SMI) se stávají rodiči v podobné míře jako neklinická populace. Jejich děti (children of parents with severe mental illness, COPMI) čelí značnému množství stresorů souvisejících s onemocněním rodiče, což je vystavuje zvýšenému riziku rozvoje vlastních duševních potíží. Prevence těchto psychických problémů a péče o COPMI jsou však v současném systému psychiatrické a klinicko-psychologické péče často opomíjeny. Cílem této práce bylo poskytnout teoretický přehled o rodinách, ve kterých je jeden z rodičů léčen pro SMI, analyzovat současnou praxi péče o COPMI v České republice a přehledně prezentovat dostupnou podporu pro rodiny s rodiči se SMI. V teoretické části práce jsou shrnuty současné poznatky o vlivu SMI na děti a rodinný systém jako celek, popsány základní potřeby těchto rodin a jejich zapojení do systému lékařské péče, a jsou představeny klíčové podpůrné a intervenční programy. Empirická část práce zahrnuje soubor čtyř studií zaměřených na zmapování situace rodin, ve kterých je dospělý člen léčen pro SMI. První studie poskytuje přehled prevalenčních výzkumů dětí a rodičů se SMI a metodologických přístupů použitých v zahraniční literatuře. Druhá část nabízí průběžné výsledky prevalenční studie a doplňuje teoretický výzkum o praktická doporučení pro práci s COPMI ve školním prostředí. Následující studie se věnuje vlastnímu prevalenčnímu výzkumu pacientů-rodičů hospitalizovaných s diagnózou SMI. Závěrečná studie zkoumá postoje a bariéry odborníků v oblasti psychiatrie a klinické psychologie, které se týkají zapojení dětí do péče o dospělé pacienty se SMI.

Abstract:

Patients with severe mental illness (SMI) become parents at a similar rate to the non-clinical population. Their children (children of parents with severe mental illness, COPMI) face a significant number of stressors related to the parent's illness, putting them at increased risk of developing mental health problems themselves. However, the prevention of these mental health problems and the care of COPMI are often neglected in the current system of psychiatric and clinical psychological care. The aim of this thesis was to provide a theoretical background of families in which one parent is being treated for SMI, to analyse the current practice of COPMI care in the Czech Republic, and to provide an overview of the support available for families with parents with SMI. The theoretical part of the thesis summarises current knowledge about the impact of SMI on children and the family system as a whole, describes the basic needs of these families and their involvement in the medical care system, and presents key support and intervention programmes. The empirical part of the thesis consists of a series of four studies designed to capture the situation of families in which an adult member is being treated for SMI. The first study provides an overview of prevalence research on children and parents with SMI and the methodological approaches used in the international literature. The second part presents the preliminary results of the prevalence study and complements the theoretical research with practical recommendations for working with COPMI in school settings. The following study focuses on our own prevalence research of parents who have been hospitalised with a diagnosis of SMI. The final study examines the attitudes and barriers of psychiatrists and clinical psychologists to involving children in the care of adult patients with SMI.
