The Influence of Therapeutic Rehabilitation on the Degree of Functional Independence in Patients after Hemorrhagic Stroke Resulting in a Speech Disorder

Abstract:

Cílem diplomové práce bylo zjistit, zda u pacientů po hemoragické cévní mozkové příhodě s následkem poruchy řeči dojde vlivem léčebné rehabilitace ke zlepšení funkční nezávislosti. Do studie bylo zařazeno 24 pacientů, z nichž polovinu tvořili pacienti s afázií a druhou polovinu pacienti s dysartrií. Prostředkem vyšetření byl test Funkční míry nezávislosti (Functional Independence Measure – FIM), který byl proveden po přijetí a před propuštěním pacientů. Statistické hodnocení dokázalo, že léčebná rehabilitace vedla u výzkumného souboru ke zlepšení funkční nezávislosti. Neprokázala se statisticky signifikantní korelace mezi hodnotami vstupní kognitivní složky a výstupní motorické složky testu FIM. Prokázalo se, že míra funkční nezávislosti při přijetí má významný vliv na délku hospitalizace. U afatiků bylo zjištěno statisticky významné zlepšení celkového, motorického i kognitivního skóre testu FIM. U dysartriků bylo zlepšení celkového a motorického skóre statisticky významné, ale změna kognitivního skóre byla statisticky nevýznamná. Prokázalo se, že přítomný typ řečové poruchy neovlivňuje hodnotu celkového a motorického skóre, ale na hodnotu kognitivního skóre má významný vliv. Došlo ke zlepšení hodnot u jednotlivých položek testu FIM, ale ne u všech hodnot bylo zlepšení vyhodnoceno jako statisticky významné.

Abstract:

The aim of this thesis was to find out if therapeutic rehabilitation would improve functional independence in patients after hemorrhagic stroke resulting in a speech disorder. The study comprised 24 patients, half of whom suffered from aphasia and half from dysarthria. The patients were examined using the Functional Independence Measure (FIM), once after admission and again before discharge. According to a statistical evaluation, therapeutic rehabilitation led to improved functional independence in the studied cohort. The study did not prove a statistically significant correlation between the values of the FIM input cognitive component and the output motor component. It was proved that the degree of functional independence on admission had a significant influence on the length of hospitalization. In the aphasics, a statistically significant improvement of the overall, the motor as well as the cognitive FIM score was measured. In the dysarthrics, the overall and the motor score improvement was statistically significant, while the cognitive score difference was statistically insignificant. It was proved that the type of speech disorder did not affect the overall and motor scores, but it had a significant influence on the cognitive score. While improvements were measured in the individual FIM components’ scores, not all these improvements were evaluated as statistically significant.
