Assistance to family with mentaly ill seniors

Abstract:

Cíl: Cílem kvalitativního výzkumu bylo zjistit a popsat subjektivní vnímání života rodinných pečujících, kteří mají/měli v péči seniora s duševní poruchou. Jak toto onemocnění narušuje vztahy v rodině, jaké potřeby pečující identifikují, porozumět náročnosti domácí péče a proč může selhávat. V neposlední řadě popsat možnosti podpory rodiny při péči o duševně nemocného seniora. Metodika: Výzkumný soubor tvořilo deset participantů z řad pečujících rodinných příslušníků, kteří byli záměrně vybráni. Následně proběhly nestandardizované polostrukturované rozhovory, které se uskutečnily po splnění nastavených kritérií a podepsaném informovaném souhlasu. Pořízený audiozáznam byl autenticky přepsán, analyzován a směřoval k vytvoření jednotlivých kategorií a subkategorií, ke kterým se participanti vyjadřovali. Výsledky: Domácí péče o seniora s duševní poruchou a psychiatrickými projevy je vnímána rodinnými pečujícími jako velmi psychicky a fyzicky náročná. Často jsou narušeny rodinné vztahy díky projevům této duševní nemoci. Subjektivně pečující prožívají nedostatek času na sebe, rodinu, koníčky, jsou vyčerpaní, unavení. Díky náročnosti péče zvažují i umístění nemocného do institucionální péče. Někteří pečující tuto možnost zásadně odmítají. Jako své potřeby pro zvládnutí péče pečující uváděli nejčastěji zapojení rodiny, sociální a zdravotní služby a hlavně postrádající informace ze strany odborníků, které byly nedostatečné či žádné. V podpoře a motivaci zvládání domácí péče by pečující uvítali možnosti finanční podpory, více dostupných sociálních služeb, komunikaci s pečujícími a stěžejní informovanost. Závěr: Podpora rodiny v péči o seniora s duševní poruchou je i v dnešní době bolestivým problémem. Přetrvávající nedostatečnost komplexních zdravotních a sociálních služeb, neinformovanost pečujících a nefungující systém gerontopsychiatrické péče v ČR celou problematiku jen negativně ovlivňuje.

Abstract:

Goals: Goal of the qualitative research was to examine and describe the perception of living of family caretakers who are/were taking care of a senior with dementia. How this illness affect family relations, what needs the caretakers identify, to understand the difficulties of home care giving and why it can fail. Last, but not least, to describe the possibilities of support for families who provide care for an elder person with dementia. Methods: The sample was created by 10 chosen participants (all family caretakers). Then, after filling the criteria and signing informed consent, non-standardized, partially structured interviews were conducted. All audio was authentically transcribed, analyzed and based on the information provided, categories and subcategories were created. The participants then expressed their view on those categories. Results: Taking care of an aged person suffering from dementia with psychiatric symptoms is perceived by the caretakers as very physically and mentally demanding. It is not rare, that the family relations are disturbed by symptoms of this illness. Subjectively, the caretakers suffer from lack of time for themselves, family, hobbies, they are exhausted and tired. As the care giving is exigent, they often consider institutionalizing ill relative, but others strictly deny this possibility. As far as they needs for care giving are concerning, they often quote involvement of family, social and medical services and mainly lack of information provided by specialist, which were frequently poor or none. In support and motivation, the caretakers would appreciate financial aid, more accessible social services, communication with other caretakers and as fundamental information. Conclusion: Even today, support of the family in care giving to a senior suffering from dementia is painful problem. Persisting lack of complex medical and social services, uninformed caretakers and malfunctioning gerontopsychiatric nursing system in Czech Republic are only worsening the whole situation.
