Využití resuscitační balonové endovaskulární okluze aorty (REBOA) u masivního krvácení - přehledová studie

The use of Resuscitative Balloon Endovascular Occlusion of the Aorta (REBOA) in massive bleeding - a review study

Abstract:

Úvod: V uplynulých letech bylo publikováno značné množství studií, která se začali zabývat metodou resuscitační endovaskulární okluzí aorty a jejím využitím při masivní krvácení. Doposud existují četné studie, které jsou však velmi často protichůdné až kontroverzní. Cílem této práce bylo zmapovat dostupné informace o užití resuscitační endovaskulární okluze aorty u pacientů s masivním krvácením a porovnat efektivitu této metody s rutinními postupy. Metody: Pro ověření stanoveného cíle byly využity výsledky analýzy dostupných původních vědeckých prací. Použita byla definovaná výběrová/vylučující kritéria a vyhledávací strategie ve třech databázích (Bibliographia medica Čechoslovaca, MEDLINE Complete, EBSCO - Academic Search Complete a CINAHL Plus with Full Text). Výsledek: Celkem bylo nalezeno 3979 zahraničních a 21 českých studií. Do přehledové práce bylo použito celkem 28 relevantních dohledaných studií. Srovnáním REBOA a resuscitační thorakotomie se zabývalo celkem 6 studií, přičemž ve čtyřech případech měla REBOA lepší výsledky z hlediska mortality než RT. Výskyt komplikací byl taktéž nižší u REBOA. Použití REBOA u masivní krvácení do pánevního prostoru se z dostupných zdrojů jeví méně efektivní ve srovnání s klasickou metodou - peritoneální balení. REBOA je zatížena výskytem velkého množství komplikací. Jako nejfrekventovanější byly zaznamenány ischémie končetiny s následnou amputací, vznik pseudoaneuryzmatu nebo reverzibilní akutní selhání ledvin. Současně bylo doloženo, že při jejím podání je zvýšené množství podaných krevních derivátů. Největší počet studií (16) se zabýval vznikem komplikací a mortalitou spojovány právě s metodou REBOA, kdy výsledky nejsou jednoznačné. Pro definitivní závěry je nutné další robustní randomizované studie. Závěr: Resuscitační endovaskulární okluze aorty se zdá jako efektivní alternativní metoda pro dočasnou zástavu krvácení, která za jistý okolností může mít pro pacienta významný benefit. Stále však panují nejednotné závěry ohledně mortality pacientů v souvislosti s využitím této metody. Výskyt komplikací není frekventní, avšak jestliže už ke komplikaci dojde, častou se jedná o komplikaci závažnou.

Abstract:

Introduction: In recent years, a considerable number of studies have been published that began to deal with the method of resuscitation endovascular occlusion of the aorta and its use in massive bleeding. So far, there are numerous studies, but they are very often contradictory and even controversial. The aim of this work was to map the available information on the use of resuscitative endovascular occlusion of the aorta in patients with massive bleeding and to compare the effectiveness of this method with routine procedures. Methods: To verify the set objective, the results of the analysis of available original scientific works were used. Defined selection/exclusion criteria and a search strategy were used in three databases (Bibliographia medica Čechoslovaca, MEDLINE Complete, EBSCO - Academic Search Complete and CINAHL Plus with Full Text). Result: A total of 3979 foreign and 21 Czech studies were found. A total of 28 relevant researched studies were used in the review work. A total of 6 studies compared REBOA and resuscitative thoracotomy, and in four cases REBOA had better mortality outcomes than RT. The incidence of complications was also lower with REBOA. From the available sources, the use of REBOA for massive bleeding in the pelvic space appears to be less effective compared to the classic method - peritoneal packing. REBOA is burdened by the occurrence of a large number of complications. Limb ischemia with subsequent amputation, formation of a pseudoaneurysm or reversible acute renal failure were recorded as the most frequent. At the same time, it was documented that during its administration, the amount of administered blood derivatives is increased. The largest number of studies (16) dealt with the emergence of complications and mortality associated with the REBOA method, when the results are not clear-cut. Further robust randomized trials are required for definitive conclusions. Conclusion: Resuscitative endovascular occlusion of the aorta seems to be an effective alternative method for temporary stoppage of bleeding, which in certain circumstances can have a significant benefit for the patient. However, there are still inconsistent conclusions regarding patient mortality associated with the use of this method. The occurrence of complications is not frequent, but if a complication does occur, it is often a serious complication.
