RANO criteria for evaluation of treatment response in high-grade gliomas - clinical and research

Abstract:

Nádory centrálního nervového systému – mozku a míchy – představují poměrně nehomogenní skupinu onemocnění. Dělí se na primární, které vznikají přímo v mozku, a na sekundární, které jsou projevem diseminace onkologického onemocnění z jiných orgánů. Sekundární tumory, metastázy, jsou výrazně četnější. Nejčastěji metastazují nádory prsu, plic, ledvin a melanomy. Primární nádory CNS nejčastěji vycházejí z podpůrné mozkové tkáně – buněk glie. Primární i sekundární mozkové nádory mozku vykazují různorodé klinické projevy závislé od jejich uložení a velikosti a představují výrazné klinické a společenské problémy pro pacienty a jejich blízké. Nejčastěji se vyskytují po 60. roce života. Incidence primárních nádorů mozku je v České republice 8 případů/100 000 obyvatel. Obsahem této práce je shrnutí obecných vlastností gliomů, jejich diagnostika a možnosti terapie, dále vyhodnocování výsledků úspěšnosti léčby a terapeutických postupů. Výraznou úlohu jak v diagnostice, tak v terapeutických postupem hrají neuroradiologické zobrazovací metody. Primární mozkový tumor z gliální mozkové tkáně – glioblastom – je nejagresivnější gliom s progresivním chováním a nepříznivou prognózou. Hlavní překážkou v efektivní léčbě glioblastomu je jeho invazivní růst do peritumorózní mozkové tkáně limitující rozsah resekce a intratumorózní heterogenita oslabující efekt onkologické terapie, přičemž radikalita chirurgické resekce je zásadní pro vývoj následného průběhu onemocnění. Další léčebné postupy navazují na chirurgický výkon. Reziduální onemocnění v peritumorózní mozkové zóně léčíme na základě analýzy tkáně masy tumoru, která byla odstraněna při operaci. V následující strategii léčby je významné nastavení kritérií hodnocení léčebné odpovědi v zobrazovacích metodách, které v poslední době umožňují využitím nových poznatků včetně umělé inteligence, přesněji a detailněji určit změny, k nimž došlo v rámci terapie v oblasti resekce a okolí. Cílem sledování je možnost co nejdříve reagovat na přítomnost časné recidivy nádoru nebo na výskyt změn spojených s terapií, zejména pseudoprogrese a poradiační nekrózy. Vývoj hodnoticích kritérií koresponduje s vývojem zobrazovacích metod i s novými znalostmi molekulárních a histopatogických vlastností nádorů mozkové tkáně. Hodnoticí kritéria léčebné odpovědi prošla vývojem v závislosti na rozvoji poznatků o imunohistopatologické povaze nádorů a rovněž vývoji v zobrazovacích metodách. Zásadním krokem se stalo rutinní zobrazování magnetickou rezonancí. Současné trendy hodnocení se snaží odrážet a rozvíjet výše uvedené faktory. V současnosti jsou k hodnocení léčebné odpovědi high-grade gliomů doporučovaná RANO hodnoticí kritéria (Response assessment in neuro-oncology criteria). Byla stanovena v roce 2010, jako snaha o preciznější stanovení stavu onemocnění, nicméně do rutinní klinické praxe se dostávají spíše pozvolně. Cílem tohoto sdělení je podpořit použití kritérií a zdůraznit jejich benefit pro další strategii léčby a rutinní využití ve výzkumu komentováním předchozích prací autorky týkajících se nádorů CNS.

Abstract:

Tumours of the central nervous system (CNS) – brain and spinal cord represent a relatively inhomogeneous group of diseases. Their basic division is into primary, which arise directly in the brain, and secondary, which are a manifestation of dissemination of cancer from other organs. Secondary tumours, metastases, are significantly more often arising mostly from breast, lung, kidney and melanoma tumours. Primary CNS tumours most often arise from the supporting brain tissue-glial cells. Both primary and secondary brain tumours have diverse clinical manifestations depending on their location and size and present significant clinical and social problems for patients and their family. They occur most frequently after the age of 60. The incidence of primary brain tumours in the Czech Republic is up to 8 patients per 100 000 inhabitants. The aim of this paper is to summarize of the general characteristics of gliomas, their diagnosis and therapeutic options, as well as the evaluation of the results of successful treatment and therapeutic procedures. Neuroradiological imaging methods play an important role here. Primary glial brain tumour glioblastoma is the most aggressive glioma with progressive behaviour and unfavourable prognosis. The main obstacle in the effective treatment of glioblastoma is its invasive growth into the peritumoral brain tissue limiting the extent of resection and intratumoral heterogeneity weakening the effect of cancer therapy, while the radicality of surgical resection is crucial for the development of the further course of the disease. Other therapeutic procedures follow the surgical procedure. Residual disease in the peritumoral brain zone is treated based on tissue analysis of the tumour mass that was removed at surgery. In the following treatment strategy, the setting of criteria for the assessment of treatment response in imaging is important, as recently the use of new knowledge, including artificial intelligence, has allowed more accurate and detailed determination of the changes that have occurred in the resection area and surrounding area during therapy. The aim of follow up is to be able to respond as early as possible to the presence of early tumour recurrence or the occurrence of therapy-related changes – in particular pseudoprogression and necrosis. The development of evaluation criteria corresponds to the evolution of imaging methods and new knowledge of molecular and histopathological characteristics of brain tumours. The evaluation criteria for treatment response have evolved in response to advances in knowledge of the immunohistopathological nature of tumours as well as developments in imaging methods. Routine magnetic resonance imaging has become a major step. Current trends in assessment seek to reflect and develop the above factors. Currently, the Response assessment in neuro-oncology criteria (RANO) are recommended to assess treatment response of high-grade gliomas. These were established in 2010 in an attempt to more accurately determine disease status, but have slowly entered routine clinical practice. The aim of this thesis is to promote the use of the criteria and highlight their benefit for further treatment strategy and routine use in research by presentation of previously publish work related to these issues.
