Optimalization of children s amblyopia refractive errors screening and dertermination of the most appropriate methodology of the comprehensive amblyopia treatment

Abstract:

Práce prokazuje bezpečnost a efektivitu nové autorkou navržené metody barevné šachovnicové reverzační stimulace sítnice (BRS) v terapii amblyopie. Byly hodnoceny změny nejlépe korigované zrakové ostrosti v souboru dětských pacientů. Srovnávána byla běžně užívaná pleoptická metoda CAM stimulátoru s novou metodou BRS. Do souboru bylo zařazeno 122 dětí s diagnosou bilaterální amblyopie, s nejlépe korigovanou zrakovou ostrostí ( NKZO ) shodnou na obou očích, sledovaných na Dětské oční klinice LF MU a FN Brno v rozmezí 10/2008 – 8/2013. Během této doby byli ze souboru všech pacientů vybráni ti, jež měli shodnou počáteční NKZO při shodné nebo jen s mírně se lišící (do 0,5 sph Dpt., do 0,5 Cyl Dpt.) refrakční vadě na obou očích. U všech účastníku studie s bilaterálně shodným stupněm amblyopie byla odebrána anamnéza prvního záchytu a tíže amblyopie. Zraková ostrost byla vyšetřována na optotypových tabulích dle věku za standardních podmínek při vyšetřovací vzdálenosti 5 metrů. U všech pacientů bylo provedeno komplexní oftalmologické i strabologické vyšetření a vyšetřen ortoptický status. Průměrný věk v době započetí terapie ve sledovaném i kontrolním souboru byl v rozmezí 3-7 let (průměr 4,9 roku, SD ± 1,29), tedy 59 měsíců (SD ± 15m). Ve sledovaném souboru pacientů bylo vždy pravé oko léčeno metodou CAM a levé metodou barevné reverzační šachovnicová stimulace BRS a to po celou dobu sledování. Efekt léčebné metody (BRS/CAM) na výsledný visus byl hodnocen ve vztahu ke zkoumaným parametrům (délka léčby, věk při zahájení léčby, visus před léčbou) pomocí analýzy rozptylu (ANOVA). Vzhledem k párovému charakteru studie byla použita ANOVA pro opakovaná měření (Repeated measure ANOVA). Vzhledem k charakteru a počtu dat a možnému nelineárnímu vztahu hodnocených parametrů k výslednému visu byly parametry kategorizovány přibližně na třetiny (podle třetinových kvantilů). V modelech pak byly hodnoceny jako kategoriální proměnné (faktory) se třemi úrovněmi odpovídajícím přibližně 1., 2. a 3. třetině hodnot. Bylo prokázáno statisticky signifikantní zlepšení NKZO ve skupině dětí léčených oběma metodami ve srovnání visu před léčbou a po léčbě. V souboru dětí léčených metodou BRS bylo výsledné zlepšení visu po terapii statisticky významné do úrovně vstupního visu 0,67 ( 5/7,5) oproti souboru léčených metodou CAM ( p<0.001) Studie potvrzuje, že stimulací sítnice barevnou reverzační stimulací došlo jednoznačně ke zlepšení nejlépe korigované zrakové ostrosti u všech dětí v našem souboru. Vývoj a vylepšení této metody spolu s důsledným screeningovým záchytem amblyogenních vad umožní předejít handicapu dětských pacientů do dospělosti.

Abstract:

This work supports the safety and effectiveness of the author’s new proposed method of colour checkerboard reversal stimulation of the retina (BRS) in the treatment of amblyopia. Evaluated were the changes in the best corrected visual acuity in a group of children. A comparison was made between the commonly used pleoptic CAM stimulator methods with the new BRS method. The group of children consisted of 122 children diagnosed with bilateral amblyopia, with best-corrected visual acuity (BCVA) identical in both eyes, under treatment at the Department of Ophthalmology for Children, University Hospital of Brno, between 10/2008 and 8/2013. During the time, the group was selected from a set of patients that had the same initial BCVA with the same or slightly differing (from 0,5 sph Dpt., to 0,5 cyl Dpt.) refractive error in both eyes. Anamnesis and the severity of amblyopia at first detection was conducted with all study participants with the same degree of bilateral amblyopia. Visual acuity was examined on optotype tables according to age under standard conditions at an examinational distance of 5 meters. All patients underwent complete ophthalmological examinations, strabological examinations and determination of orthoptic status. The average age at the beginning of therapy in the reference and control groups where in the range of 3-7 years (average 4.9 year ± 1.29), 59 months (± 15 m). The reference group of patients always had the right eye treated using the CAM method and their left eye treated using the colour checkerboard reversed stimulation BRS method and were monitored throughout the entire period of treatment. The effect of the (BRS/CAM) therapeutic method on the resulting visus was evaluated in relation to the parameters examined (length of treatment, age at beginning of treatment, visus prior to treatment) by analysing variance (ANOVA). Due to the paired nature of the studies, ANOVA was used for repeated measurements (Repeated measure ANOVA). Due to the character, amount of data and possible non-linear relationship of the evaluated parameters to the resulting visus, the parameters were categorized roughly in thirds (according to one-third quintiles). In the models they were then evaluated as categorical variables (factors) with three levels corresponding to approximately 1st, 2nd and 3rd thirds of the values. Statistically significant improvement was demonstrated in BCVA in the group of children treated using both methods in comparing visus before and after treatment. In the group of children treated using the BRS method, the resulting visus improvement after therapy was statistically significant up to the entry visus level of 0,67 (5/7,5) in comparison to the group treated using the CAM method (p < 0.001). This study confirms that stimulation of the retina using colour reversing stimulation was clearly seen to improve the best corrected visual acuity of all children in the study group. Development and improvement of this method together with rigorous screening for and detection of amblyopic defects can lead to prevention of the handicap carrying over to adulthood in paediatric patients.
