Abstract:

Úvod: Dilatační kardiomyopatie (DCM) je velmi častým onemocněním vedoucím k srdečnímu selhání. Etiologie a klinický průběh tohoto onemocnění jsou velmi heterogenní a patofyziologické procesy které jsou podkladem DCM na molekulární úrovni stále nejsou podrobně prozkoumány. V této disertační práci popisujeme dva přístupy ke studiu genetické architektury DCM a patofyziologických mechanismů vedoucích k fenotypu srdečního selhání. V práci Genetická architektura recentně vzniklé dilatační kardiomyopatie v Moravském regionu za využití celo-exomového sekvenování a její klinické koreláty bylo naším cílem popsat genetickou architekturu recentně vzniklé dilatační kardiomyopatie prostřednictvím celo-exomového sekvenování (WES) a studovat korelaci genotypu a reverzní remodelace levé komory srdeční (LVRR) ve dvanáctiměsíčním sledování. Ve studii Profil transkripčních faktorů u idiopatické dilatační kardiomyopatie představujeme využití vysoce citlivé platformy zvané Quanttrx. Jedná se o první popis profilu exprese všech lidských transkripčních faktorů (TF) pomocí této technologie. Dále byl zkoumán rozdíl v expresi jednotlivých transkripčních faktorů u pacientů s DCM ve srovnání se zdravými jedinci a byla studována role těchto rozdílně vyjádřených TF v patofyziologii tohoto onemocnění. Metodika: Genetická architektura recentně vzniklé dilatační kardiomyopatie v Moravském regionu za využití celo-exomového sekvenování a její klinické koreláty V této multicentrické prospektivní observační studii, která byla prováděna primárně na I. interní kardioangiologické klinice FN u sv. Anny v Brně bylo zařazeno 83 pacientů z Moravského regionu s recentně zjištěnou DCM (symptomy méně než 6 měsíců) pro celo-exomové sekvenování (WES). Všichni pacienti postoupili 12 měsíční klinické a echokardiografické sledování. LVRR byla definována absolutním nárůstem ejekční frakce levé komory> 10 % a relativním zmenšením levokomorového end-diastolického rozměru ˃ 10 % ve 12 měsících. Profil transkripčních faktorů u idiopatické dilatační kardiomyopatie Tato multicentrická prospektivní observační studie byla provedena v Beth Israel Deaconess Medical Center – Harvard University v Bostonu v USA a byla součástí šestiměsíčního výzkumného pobytu na tomto pracovišti, který byl integrální součástí mého doktorského studia. Vzorky myokardu od pacientů s DCM a kontrolních subjektů byly analyzovány za použití vysoce senzitivní platformy Quanttrx, která je založena na kvantitativní polymerázové řetězové reakci v reálném čase (qRT-PCR). Následně byly pomocí bioinformatických metod studovány molekulární a buněčné funkce a dráhy diferenciálně vyjádřených TF. Výsledky: Genetická architektura recentně vzniklé dilatační kardiomyopatie v Moravském regionu za využití celo-exomového sekvenování a její klinické koreláty WES identifikoval alespoň jednu patogenní genetickou variantu u 45 pacientů (54 %). LVRR nastala u 28 pacientů (34 %), nejčastěji u nosičů izolovaných trunkujících variant titinu, následovaných pacienty s negativním nebo nekonkluzivním výsledkem WES, a dále nosiči jiných patogenních variant (56 % vs. 42 % vs. 19 %, P=0,041). Profil transkripčních faktorů u idiopatické dilatační kardiomyopatie Zjistili jsme 39 diferenciálně vyjádřených TF. Z tohoto celkového počtu 39 TF byla ve skupině pacientů s DCM u 17 TF (44 %) exprese zvýšena (fold-change ≥ +2, P <0,05), zatímco u 22 TF (56 %) byla snížena (fold-change ≥-2, P <0,05) ve srovnání s dárci. Analýza odhalila TF začleněné do známých signalizačních drah hrajících roli v srdečním vývoji a onemocnění. ačují dosud neprozkoumané vazby mezi biologickými procesy odpovědnými za srdeční degeneraci a stárnutí v kontrastu s narušenou hyperproliferací a růstem. Závěr: Genetická architektura recentně vzniklé dilatační kardiomyopatie v Moravském regionu za využití celo-exomového sekvenování a její klinické koreláty Velká část pacientů s recentně vzniklou DCM má monogenní či oligogenní genetické pozadí. Nosiči netitinových patogenních variant mají menší pravděpodobnost dosažení reverzní remodelace levé komory ve 12 měsících ve srovnání s ostatními. Genetické testování může přispět k lepší predikci prognózy a individualizované léčbě pacientů s recentně vzniklou DCM. Profil transkripčních faktorů u idiopatické dilatační kardiomyopatie Naše vysoko-objemová platforma Quanttrx umožnuje rychlý a efektivní screening všech lidských TF a představuje cenný nástroj pro další stadium molekulárních mechanismů DCM a dalších onemocnění. Naše poznatky popisují profil změn exprese TF u pacientů s DCM, ale především identifikují nové biologické dráhy, které mohou sloužit jako potencionální diagnostické a léčebné cíle.

Abstract:

Aims: Dilated cardiomyopathy (DCM) is a prevalent disease leading to heart failure. It has a heterogeneous aetiology and clinical outcomes and the pathophysiology of the disease on molecular level is still poorly understood. In this thesis we present two different approaches to study the genetic architecture of DCM and pathophysiological mechanisms leading to failing phenotype. In the study Genetic architecture of recent-onset dilated cardiomyopathy in Moravian region assessed by whole-exome sequencing and its clinical correlates we aimed to assess the genetic architecture of recent-onset DCM by whole-exome sequencing and correlate genotype with left ventricular reverse remodelling (LVRR). The study Human Transcription Factor Profiling of Idiopathic Dilated Cardiomyopathy aimed to introduce a highly-sensitive platform Quanttrx to first describe the expression profile of all human transcription factors (TFs) using this technology. Furthermore, we aimed to identify the differentially expressed TFs in DCM compared to healthy controls and study their role in the pathogenesis of the disease. Methods: Genetic architecture of recent-onset dilated cardiomyopathy in Moravian region assessed by whole-exome sequencing and its clinical correlates In this multi-centre prospective observational study, performed primarily at Ist internal cardioagiology department of St. Anne’s University Hospital in Brno Czech Republic, we enrolled 83 Moravian patients with recent-onset DCM and a history of symptoms of less than 6 months, for whole-exome sequencing (WES). All patients underwent 12-month clinical and echocardiographic follow-up. LVRR was defined as an absolute increase in left ventricular ejection fraction > 10% accompanied by a relative decrease of left ventricular end-diastolic diameter ˃ 10% at 12 months. Human Transcription Factor Profiling of Idiopathic Dilated Cardiomyopathy This multi-centre prospective observational study was performed at Beth Israel Deaconess Medical Center – Harvard University in Boston USA as part of a 6-month research stay which was an integral part of this doctoral study. Myocardial tissue samples from DCM and control subjects were analysed using highly sensitive screening platform, named Quanttrx, which is based on quantitative real-time polymerase chain reaction (qRT PCR). The differentially expressed genes were then analysed using bioinformatic methods to assess their molecular and cellular functions and pathways. Results: Genetic architecture of recent-onset dilated cardiomyopathy in Moravian region assessed by whole-exome sequencing and its clinical correlates WES identified at least one disease-related variant in 45 patients (54%). LVRR occurred in 28 patients (34%), most often in carriers of isolated titin truncated variants, followed by individuals with a negative, or inconclusive WES and carriers of other disease-related variants (56% vs. 42% vs. 19%, P=0.041). Human Transcription Factor Profiling of Idiopathic Dilated Cardiomyopathy We identified 39 differentially expressed TFs. 17 TFs (44 %) were upregulated (fold change >= 2, P < 0.05) while 22 (56 %) were downregulated (fold change >= 2, P < 0.05) in DCM. The analysis uncovered TFs affecting known signalling pathways involved in cardiac development and disease. Importantly, our analysis identified novel regulatory pathways that suggest intriguing unexplored antithetical associations between biological processes governing cardiac degeneration and aging vs. disordered hyper-proliferation and growth. Conclusion: Genetic architecture of recent-onset dilated cardiomyopathy in Moravian region assessed by whole-exome sequencing and its clinical correlates A substantial proportion of recent-onset DCM cases have a monogenic or oligogenic genetic background. Human Transcription Factor Profiling of Idiopathic Dilated Cardiomyopathy Our high-throughput platform Quanttrx allows quick and effective screening of all human TFs providing a valuable resource for further investigation of the molecular mechanisms of DCM as well as other diseases. Our data provides a profile of TFs expression changes in DCM patients, but also, importantly, identifies novel biological pathways that may serve as potential diagnostic or therapeutic targets.
