Analysis of microRNAs in atypical meningioma patients

Abstract:

Atypické meningeomy tvoří okolo 18 % všech meningeomů a vyznačují se agresivnější povahou. Nejčastěji jsou léčeny chirurgickou resekcí, avšak k relapsu onemocnění dochází zhruba u 50 % případů, a to i po kompletním odstranění nádoru. Z tohoto důvodu je pacientům po chirurgické resekci často indikována adjuvantní radioterapie. I přes významné pokroky v diagnostice je stále obtížné predikovat, u jakých pacientů hrozí po resekci zvýšené riziko rekurence nádoru a kteří pacienti by mohli mít prospěch z adjuvantní radioterapie. V posledních letech se vědecká pozornost obrátila i na nekódující RNA molekuly, zejména mikroRNA (miRNA), vzhledem k jejich potenciálu sloužit jako prognostické a prediktivní biomarkery. MiRNA se řadí mezi posttranskripční regulátory genové exprese a jsou zapojeny v klíčových biologických procesech, včetně buněčné proliferace, diferenciace, apoptózy. Změna jejich exprese přispívá k patogenezi mnoha malignit, včetně meningeomů. Studie také ukázaly jejich potenciál predikovat odpověď na léčbu u atypických meningeomů. Cílem této práce bylo s využitím vysokokapacitních metod identifikovat miRNA schopné predikovat časný relaps a odpověď na adjuvantní radioterapii u atypických meningeomů a následně vybrané miRNA validovat pomocí metody RT-qPCR. Dalším cílem bylo prostudovat expresní profil miRNA u pacientů s indikovanou adjuvantní radioterapií bez rekurence a pokusit se stratifikovat pacienty, kteří by pravděpodobně nebyli v riziku časného relapsu i při absenci adjuvantní léčby a u nichž tedy rizika terapie převyšují očekávaný benefit. Výsledkem této práce byla identifikace miR-99b-3p, která vykazuje potenciál fungovat jako prediktivní biomarker odpovědi na adjuvantní radioterapii a miR-214-3p s potenciálem predikovat relaps do 5 let u pacientů s atypickým meningeomem. Avšak na základě dalších provedených analýz se bohužel nepodařilo sestavit dostatečně robustní panel miRNA schopný jednoznačně definovat pacienty, kterým by nemusela být indikována adjuvantní radioterapie.

Abstract:

Atypical meningiomas account for approximately 18% of all meningiomas and exhibit a more aggressive nature. They are most commonly treated through surgical resection; however, disease relapse occurs in about 50% of cases, even after complete tumor removal. Therefore, adjuvant radiotherapy is often indicated for patients following surgical resection. Despite significant advancements in diagnosis, it remains challenging to predict which patients are at an increased risk of tumor recurrence after resection and which patients would benefit from adjuvant radiotherapy. In recent years, scientific attention has also turned to non-coding RNA molecules, particularly microRNAs (miRNAs), due to their potential as prognostic and predictive biomarkers. MiRNAs are classified as post-transcriptional regulators of gene expression and are involved in essential biological processes, including cell proliferation, differentiation, and apoptosis. Changes in their expression contribute to the pathogenesis of many malignancies, including meningiomas. Studies have also demonstrated their potential to predict treatment response in atypical meningiomas. This study aimed to utilize high-capacity methods to identify miRNAs capable of predicting early relapse and response to adjuvant radiotherapy in atypical meningiomas and subsequently validate selected miRNAs using the RT-qPCR method. Another goal was to examine the miRNA expression profile in patients with indicated adjuvant radiotherapy without recurrence and attempt to stratify patients who would likely not be at risk of early relapse without adjuvant therapy, thus outweighing the risks of treatment with the expected benefits. The results of this study identified miR-99b-3p, which exhibited potential as a predictive biomarker for the response to adjuvant radiotherapy, and miR-214-3p, which showed potential for predicting relapse within five years in patients with atypical meningioma. However, based on further analyses, it was unfortunately impossible to construct a sufficiently robust miRNA panel capable of definitively identifying patients who would not require adjuvant radiotherapy.
