Survival of cancer patients controlled for potential confounders based on NHIS data

Abstract:

Relativní přežití pacientů s onkologickým onemocněním je užitečným ukazatelem závažnosti onemocnění a úspěšnosti léčby nebo screeningových programů. Často se využívá ke srovnání skupin pacientů, např. geografických. V takových případech bývá standardizováno pro věk, případně věk a stádium. Porovnávané skupiny se však mohou lišit i v dalších faktorech. Příkladem může být přítomnost dalších onemocnění, které nejen, že pacienta oslabují, ale také mohou být důvodem pozdní detekce onkologického onemocnění a znemožnit aplikaci vhodné léčby. Tato práce se zabývá hodnocením relativního přežití pacientů s pěti vzorovými skupinami onkologických diagnóz v České republice na základě dat Národního onkologického registru. V datech dalších registrů Národního zdravotnického informačního systému (NZIS) jsou identifikovány faktory, které by mohly ovlivňovat přežití pacientů a srovnatelnost jejich skupin. Informace o výskytu dalších onemocnění je hodnocena ve formě jednotlivých onemocnění, ale i ve formě komorbiditního indexu, dále jsou zahrnuty i informace o onkologické léčbě. Tyto faktory jsou následně porovnány pomocí regresních modelů relativního přežití a na základě výsledků těchto modelů jsou vybrány faktory vstupující do výpočtu adjustovaného pětiletého relativního přežití v krajích. Za vhodné pro adjustaci jsou zvoleny onkologická léčba, léčba v Komplexně onkologickém centru (KOC) a Deyova modifikace komorbiditního indexu Charlsonové (DCCI). Rozdíly v přežití mezi kraji po zahrnutí těchto faktorů v některých případech klesají, což naznačuje, že tyto faktory vysvětlí část variability. U jiných, např. u novotvarů slinivky, je mezi kraji po adjustaci na tyto faktory větší rozdíl. Tento výsledek poukazuje na nutnost hledat pro tyto rozdíly jiné vysvětlení.

Abstract:

The relative survival of patients with oncological diseases is a useful indicator of the severity of the disease and the effectiveness of treatment or screening programs. It is often used to compare groups, such as geographical ones. In such cases, it is standardized for age, and possibly age and stage. However, the compared groups may differ in other factors as well. An example could be the presence of other diseases, which not only weaken the patient but can also be a reason for late detection of oncological disease and prevent the application of appropriate treatment. This thesis deals with the evaluation of relative survival of patients with five model groups of oncological diagnoses in the Czech Republic. In the data from the National Health Information System (NHIS), factors that could influence survival are identified. Information about the occurrence of other diseases is evaluated in the form of individual diseases, but also in the form of the comorbidity index, and information about oncological treatment is included as well. These factors are then compared using regression models of relative survival, and based on the results of these models, factors entering into the calculation of adjusted five-year relative survival in the regions are selected. The following factors have been chosen as suitable for adjustment: oncological treatment, treatment in a Comprehensive Oncological Center (COC), and Deyo's modification of the Charlson Comorbidity Index (DCCI). Differences in survival between regions after accounting for these factors sometimes decrease, suggesting that these factors explain part of the variability. In other cases, such as with pancreatic tumors, there is a larger difference between regions after adjusting for these factors. This result indicates the need to seek other explanations for these differences.
