Quantification of SARS-CoV-2 nucleocapsid antigen: antigenic dynamics and relationship to evidence of SARS-CoV-2 in respiratory samples.

Abstract:

At the end of 2019, cases of pneumonia caused by the newly discov-ered SARS-CoV-2 virus appeared in China. The disease caused by the virus was named COVID-19 and within a few months it spread all over the world. Its typical manifestations are fever, dry cough, general ex-haustion, and muscle pain, but in severe cases the infection can be fa-tal. One of the key aspects for effective control and limiting the spread of the virus was rapid and accurate diagnosis, which required the de-velopment of adequate diagnostic tests. One of the possibilities for monitoring the course of infection is the determination of nucleocapsid antigen from peripheral blood. This work focuses on monitoring the dynamics of N-antigenemia in a selected group of patients who had detectable concentration of N-antigen in peripheral blood determined by ELISA test and at the same time had SARS-CoV-2 infection confirmed by RT-PCR method. Our goal was to compare the observed facts with the data found in the litera-ture. The results showed it is possible to effectively monitor the dynam-ics of antigenemia with this test and the N-antigen level reaches its peak between the 3rd and 10th day after a positive PCR test in most patients. Considering previously published studies, the main difference in our work is having the day of PCR positive result as a starting point.

Abstract:

V závere roka 2019 sa v Číne objavili prípady pneumónie spôso-bené novoobjaveným vírusom SARS-CoV-2. Ochorenie ním spôsobené bolo nazvané COVID-19 a v priebehu niekoľkých mesiacov sa rozšírilo do celého sveta. Jeho typickými prejavmi sú teplota, suchý kašeľ, celko-vá vyčerpanosť a bolesti svalov, ale vo vážnych prípadoch môže byť infekcia smrteľná. Jedným z kľúčových aspektov k efektívnej kontrole a k obmedzeniu šírenia vírusu bola rýchla a presná diagnostika, ktorá vyžadovala vývin adekvátnych diagnostických testov. Jednou z možností ako sledovať priebeh infekcie predstavuje stanovenie nuk-leokapsidového antigénu z periférnej krvi. Táto práca sa zaoberá sledovaním dynamiky N-antigenémie u vybranej skupiny pacientov, ktorým bola prostredníctvom metódy ELISA stanovená koncentrácia N-antigénu v periférnej krvi a zároveň mali títo pacienti potvrdenú infekciu SARS-CoV-2 metódou RT-PCR. Našim cieľom bolo porovnanie pozorovaných skutočností s údajmi zistenými v použitých štúdiách a vyhodnotenie miery zhody našich vý-sledkov s naštudovanou literatúrou. Výsledky nám ukázali, že tento test je vhodný na sledovanie dy-namiku antigenémie a hladina N-antigénu dosahuje u väčšiny pacien-tov vrchol medzi 3. a 10. dňom od pozitívneho PCR testu. Vzhľadom k použitej literatúre je v našej práci zásadným rozdielom stanovenie prvého dňa PCR pozitivity ako východiskového bodu.
