Security process to prevent patient confusion

Abstract:

Cíl: Cílem práce bylo zjistit, jak je prováděn bezpečnostní proces na operačních sálech vybraného zdravotnického zařízení. Metodika: Sběr dat byl prováděn metodou pozorování personálu pracujícího na operačních sálech gynekologicko - porodnické kliniky vybraného zdravotnického zařízení. Pozorování probíhalo při všech třech fázích bezpečnostního procesu. Byly provedeny zápisy šetření do auditního listu zdravotnického zařízení, který byl doplněn vlastními dodatky o věku pacientek a o provedení třetí fáze bezpečnostního procesu. Tento auditní list byl sestaven z otázek, které jsou sestaveny ze standardu zdravotnického zařízení - Univerzální protokol prevence záměny pacienta, místa operace či prováděného výkonu. Počet invazivních výkonů, které byly vybrány náhodně v období od 1. 6. 2020 do 31. 12. 2020, kdy auditní šetření probíhalo, byl 120. Výsledky: První fáze bezpečnostního procesu, kdy je pacient předáván mezi sestrou z oddělení a anesteziologickou sestrou byla splněna u všech výkonů plánovaných i akutních. Jedná se o identifikaci pacienta, předání jeho dokumentace a zápis do tiskopisu bezpečnostního procesu. Druhou fázi bezpečnostního procesu, která spočívá v identifikaci výkonu, strany a celkové připravenosti operačního týmu těsně před počátkem operace provádí perioperační sestra. Tato druhá fáze nebyla splněna u dvou urgentních výkonů. Třetí fáze bezpečnostního procesu, která se provádí po ukončení výkonu, byla splněna ve všech 120 případech. Závěr: Auditním šetřením bylo zjištěno, že ve vybraném zdravotnickém zařízení je bezpečnostní proces prováděn zaměstnanci ve všech třech fázích a dle platného ošetřovatelského standardu. Odchylkou je neprovedení druhé fáze bezpečnostního procesu u urgentních operačních výkonů, kdy se veškerá pozornost operačního týmu soustřeďuje k záchraně života jak pacientky, tak k co nejrychlejšímu vybavení plodu a jeho záchraně.

Abstract:

Aim: The aim of the thesis was to find out how the security process is performed in the operating rooms of a selected medical facility. Methodology: Data collection was performed by the method of observation of staff members working in the operating rooms of the gynecological-obstetrics clinic of a selected medical facility. The observation was running during all three phases of the security process. The records of the inquiry were made in the audit sheet of the medical facility, which was supplemented by own additions regarding the age of female patients and implementation of the third phase of the safety process. This audit sheet was compiled from questions that are compiled from the standard of the medical facility - Universal protocol for the prevention of patient confusion, the place of surgery or medical performance that is performed. The number of invasive performances, that were selected randomly, in the period from 1. 6. 2020 to 31. 12. 2020, when the audit investigation was running, was 120. Results: The first phase of the safety process, when the patient is transferred between the nurse from the ward and the anesthesiology nurse, was fulfilled at all planned and acute procedures. This involves identifying the patient, handing over his documentation and enrolment in the safety process form. The second phase of the security process, consisting in identifying the performance, the side and the overall readiness of the operating team just before the start of the operation, is performed by the perioperative nurse. This second phase was not fulfilled in case of two urgent procedures. The third phase of the safety process, which is performed after the termination of the performance, was fulfilled in all 120 cases. Conclusion: Through the audit investigation it was found out that in the selected medical facility, the safety process is performed by employees in all three phases and according to the valid nursing standard. The deviation is the failure to perform the second phase of the safety process at the urgent surgical procedures, where all the attention of the operating team is concentrated on saving the life of both the female patient and the fastest possible equipment of the foetus and its rescue.
