Importance of thermoregulation in patients on emergency admission

Abstract:

Úvod: Diplomová práce se snaží zpřehlednit problematiku termoregulace u pacientů v závažném stavu na urgentním příjmu. Cíle práce: Teoretickou podstatou práce je zmapovat z českých a zahraničních zdrojů informace ohledně problematiky termoregulace. Empirickým cílem je posoudit význam termoregulace u pacientů v závažném stavu na urgentním příjmu analýzou vývoje tělesné teploty a jejího vlivu na celkový outcome pacienta. Metodika: Pozorování a měření bylo realizováno prostřednictvím posuzovacího formuláře. Data byla sbírána na urgentním příjmu fakultní nemocnice se statutem traumacentra v období měsíce březen až prosinec 2021. Zápis tělesné teploty probíhal v 10minutových intervalech. Pro zařazení do studie byly zvolené následující kritéria: primární směřování, následná hospitalizace, minimální délka pobytu 45 minut a minimální věk 18 let. Následně byla data analyzována statistickým programem NCSS. Statistické testy byly hodnoceny na hladině statistické významnosti ?=0,05. Výsledky: Zkoumaný soubor byl sestaven z 274 pacientů. Nejvíce negativní hodnoty byly zaznamenány při příjmu pacienta. V průběhu ošetřování docházelo k postupnému upravování k fyziologické hranici. Výsledné hodnoty tělesné teploty se pohybovaly v požadovaných či předpokládaných parametrech. Z hlediska podrobnějšího popisu lze konstatovat, že nejzásadnější hodnoty byly identifikovány u pacientů s kardiologickou problematikou. Vstupní a výstupní hodnota tělesné teploty činila 35,6 °C. V průběhu ošetřování docházelo v určitých intervalech k vzestupu a posléze naopak k poklesu tělesné teploty. Dále bylo identifikováno, že vývoj tělesné teploty může ovlivňovat mnoho faktorů. Závěr: Znát směr vychýlení tělesné teploty lze považovat za přínosné, poněvadž včasná korekce patologických hodnot může mít stěžejní vliv na celkový outcome pacienta.

Abstract:

Introduction: The diploma thesis tries to clarify the issue of thermoregulation in patients in severe condition in the emergency admission unit. Objectives: The theoretical essence of the work is to map information from Czech and foreign sources on the issue of thermoregulation. The empirical aim is to assess the importance of thermoregulation in patients in severe condition by analyzing the development of body temperature and its effect on the patient's overall outcome. Methodology: Observations and measurements were made through an assessment form. Data were collected at the emergency admission unit of the university hospital with the status of a trauma center in the period from March to December 2021. The body temperature was recorded at 10-minutes intervals. The following criterias were chosen for inclusion in the study: primary aim, following hospitalization, minimum stay 45 minutes and minimum age 18 years. Subsequently, the data were analyzed by the statistical program NCSS. Statistical tests were evaluated at the level of statistical significance ? = 0.05. Results: The study group was consisted of 274 patients. The most negative values were recorded at the admission of the patient. During the treatment, there was a gradual shift to the physiological limit. The resulting body temperature values were within the required or expected parameters. In terms of a more detailed description, it can be stated that the most significant values were identified in patients with cardiac problems. The input and output body temperature values were 35.6 ° C. During the treatment, there was an increase and then a decrease in some intervals in body temperature measurement. It has also been identified that the development of body temperature can be influenced by many factors. Conclusion: Knowing the direction of body temperature deviation can be considered beneficial, as early correction of pathological values can have a major impact on the overall outcome of the patient.
