Abstract:

K vyšetření malé pánve magnetickou rezonancí jsou indikovány mimo jiné ženy s podezřením na maligní proces dělohy - cervixu, endometria, pochvy, vulvy atd., s metastatickým postižením lymfatických uzlin či bez. Vyšetření provádím na MR přístroji Siemens Symphony 1,5 T za použití celotělových cívek. Za užitečnou součást považuji podání kontrastní látky v tzv.dynamické studii, která umožňuje lepší diferenciaci tkání na základě odlišného prokrvení a tím sycení kontrastní látkou v čase. Kinematografická studie v čase za sebou provedených řezů dovoluje pak lépe posoudit rozhraní jednotlivých tkání, hlavně pak patologických od zdravých, nepostižených. Mimoto provádíme volumometrie nádoru v rámci dalšího zpracování získaného obrazu.

Abstract:

Meaning of dynamic MR sequentions (Dynamic contrast-enhanced magnetic resonance imaging (DCE-MRI)) in diferentiation patological and healthy tissue in oncogynaecological indications. Key words: magnetic resonance imaging, cancer of uterine cervix, cancer of endometrium, zonal differentiation, dynamic T1W MRI sequencies, to increase sensitivity Introduction: magnetic resonance examination of female pelvis in oncological indications, mainly cancers of uterine cervix and endometrium, offers the most specification thanks to excellent tissue diferentiation. Inspite of lot of malignancies in the pelvis are difficult to distinguish healthy tissue from patological tissue. Material and method: Pacients with cancer of uterine cervix (20), cancer of endometrium (17). MR examination using T2W in sagittal and axial planes, dynamic T1W sequencies after IV aplication GD, 1,5 T Siemens Symphony, body coils. Discussion : we can use dynamic MR sequentions, then we can observe various and gradual enhancement of different tissues (cancer, fibromuscular stroma, parametrium, myometrium etc.) after aplication IV bolus of kontrast media in time using very fast and short T1 weighted MR sequencies quickly after itselves, to determine interface between normal to changed tissue and then is increased sensitivity of MR examination. Outcome of this examination is maximal zonal differentiation in the MR images. The second is to specify lymph nodes of pelvis. Conclusion: it is very importan to determine precise information of tumor (extent), to specify the best kind of therapy - surgery or chemotherapy or both, to specify the best benefit for pacient.
