The efficacy of the Blood Flow Restriction method in patients with muscular hypotrophy of the musculus vastus medialis following arthroscopic surgery of the meniscus of the knee joint

Abstract:

Cílem diplomové práce bylo posoudit účinek metody Blood Flow Restriction u pacientů se svalovou hypotrofií musculus vastus medialis po artroskopické operaci kolenního kloubu. Pro ověření účinku byla u pacientů měřena svalová síla, obvod svalů, rozsah pohybu a funkční stav operované dolní končetiny. Studie se zúčastnilo 20 probandů z toho 13 můžu a 7 žen ve věku od 19 do 71 let. Pacienti byli náhodně rozděleni do skupiny podstupující metodu Blood Flow Restriction a skupiny podstupující standardní rehabilitaci po operaci kolenního kloubu. Všichni pacienti podstoupili vstupní vyšetření, 8 třicetiminutových terapií a výstupní vyšetření. Pro hodnocení svalové síly byl využit dynamometr Isomed 2000. Obvod stehna dolní končetiny byl měřen pomocí krejčovského metru. Rozsah pohybu byl měřen pomocí dvouramenného goniometru a funkční stav operované dolní končetiny pomocí Lysholmova skórovacího dotazníku. U obou skupin došlo po terapiích ke zvýšení svalové síly, obvodu svalů a rozsahu pohybu v kolenním kloubu. Funkční stav operované dolní končetiny byl taktéž u obou skupin zlepšen. Pacienti podstupující metodu BFR dosáhli vyššího průměrného zlepšení svalové síly o 33,9 Nm do flexe a 30,9 Nm do extenze než pacienti ve skupině standardní terapie. Dle Mann-Whitneyova U test byl statisticky významný rozdíl mezi skupinami pouze do flexe. Výstupní hodnoty nárůstu obvodu stehna byly zvýšeny u obou skupin. Skupina BFR dosáhla v průměru po konci terapii nárůstu o 1,8 cm. Tento výsledek je hodnocen jako statisticky významný. V porovnání s kontrolní skupinou, u které došlo k průměrnému zlepšení v obvodu stehna o 0,7 centimetrů byl výsledek skupiny BFR statisticky nevýznamný. V oblasti rozsahu pohybu kolenního kloubu došlo ke zlepšení u obou skupin. Výstupní hodnoty byly v průměru vyšší u skupiny BFR a to o 6° při aktivním pohybu a o 5°při pasivním pohybu do flexe. Průměrné zlepšení rozsahu pohybu bylo vyšší u skupiny podstupující standardní rehabilitaci, protože průměrný rozsah pohybu byl při vstupním vyšetření nižší. U obou skupin došlo k výraznému zlepšení v oblasti funkčního využití operované končetiny. Výstupní hodnoty Lysholmova skóre bylo u BFR skupiny v průměru vyšší o 2,3 bodů než u skupiny standardní terapie. Tento rozdíl nebyl dle Mann-Whitneyova U test statisticky významný. Tyto výsledky částečně ukazují rehabilitační potenciál metody BFR oproti standardní rehabilitaci u pooperačních kolenních kloubů. Pro lepší zhodnocení účinnosti metody Blood Flow Restriction je potřeba dalšího výzkumu.

Abstract:

The objective of this diploma thesis was to assess the efficacy of the Blood Flow Restriction method in patients with muscular hypotrophy of the musculus vastus medialis following arthroscopic surgery of the knee joint. Muscle strength, the circumference of muscles, the range of motion and the functional status of the operated lower limb were all measured in patients in order to verify the efficacy of this method. 20 probands took part in the study, 13 of these were men and 7 were women, aged between 19 and 71 years. The patients were randomly divided into a group undergoing the Blood Flow Restriction method and a group undergoing standard rehabilitation following surgery of the knee joint. All the patients underwent an entrance examination, 8 thirty-minute therapy sessions and an exit examination. An Isomed 2000 dynamometer was used to assess muscle strength. The circumference of the thigh in the lower limb was measured using a tailor's tape measure. The range of motion was measured using a two-arm goniometer and the functional status of the operated lower limb was assessed using a Lysholm scoring scale. Muscle strength, the circumference of the muscles and the range of motion in the knee joint increased in both groups. The functional status of the operated lower limb also improved in both groups. Patients undergoing the BFR method achieved a higher average increase in muscle strength by 33.9 Nm in flexion and 30.9 Nm in extension. According to the Mann-Whitney U test there was only a statistically significant difference between the groups in flexion. The output values of the increase in thigh circumference were higher in both groups. The BFR group achieved an increase by 1.8 cm on average by the end of therapy. This result is considered statistically significant. In comparison to the control group, in which the average increase in thigh circumference was by 0.7 cm, the results of the BFR group are statistically insignificant. The range of motion of the knee joint also improved in both groups. The output values were higher on average in the BFR group, by 6° during active movement and by 5°during passive movement into flexion. The average improvement in the range of motion was higher in the group undergoing standard rehabilitation, because the average range of motion was lower during the entrance examination. There was significant improvement in the functional use of the operated limb in both groups. The output values of the Lysholm score were higher on average by 2.3 points in the BFR group compared to the group undergoing standard therapy. This difference was not statistically significant according to the Mann-Whitney U test. These results show to some extent the rehabilitation potential of the BFR method compared to standard rehabilitation of knee joints post-surgery. Further research is necessary for better assessment of the efficacy of the Blood Flow Restriction method.
