Importance of genomics, transcriptomics and proteomics in determining medulloblastoma molecular subgroups

Abstract:

Medulloblastoma is a highly aggressive pediatric brain tumor. Despite advances in treatment, 10 % of all cancer related deaths are caused by this malignancy. Intensive treatment is necessary, however, it has negative side effects that affect patient's quality of life. Therefore a risk stratification is very important for determination of the treatment intensity. The risk is defined according to patient's age, presence of metastases, possibility of surgical resection, histopathology, etc. In last few years, methods of molecular biology are increasingly applied. These techniques allow a more detailed classification of the tumors and thus determination of the appropriate therapy. According to the transcriptome analysis there are four molecular subgroups of medulloblastoma, which can be further divided into subtypes with different prognoses. New methods of molecular biology as are next generation gene sequencing and detection of epigenetic changes are used for identification of genes involved in medulloblastoma pathogenesis. The aim of this bachelor thesis is to give a general overview of approaches used to determine medulloblastoma molecular subgroups and to describe characteristics of these subgroups, including specific identified markers and their clinical significance.

Abstract:

Meduloblastóm je vysoko agresívny detský mozgový nádor, ktorý je aj cez pokroky v liečbe stále príčinou úmrtí približne desať percent všetkých detí, ktoré umierajú v dôsledku nádorového ochorenia. Intenzívna liečba má často trvalé následky, ktoré negatívne ovplyvňujú kvalitu ďalšieho života. Je preto dôležité stanoviť u každého pacienta mieru rizika a z nej vyplývajúcu intenzitu liečby. V súčasnosti sa tak deje na základe veku pacienta, prítomnosti metastáz, možnosti chirurgického odstránenia nádoru, histopatologických charakteristík atď. V posledných rokoch sa stále viac uplatňujú aj metódy molekulárnej biológie, ktoré umožňujú podrobnejšiu klasifikáciu nádoru, a teda aj presnejšie určenie vhodnej liečby. Na základe analýzy transkriptómov boli identifikované štyri základné molekulárne podskupiny meduloblastómu, ktoré je možné ešte ďalej deliť na skupiny s rozdielnou prognózou. Pre identifikáciu génov, ktoré sú zapojené v patogenéze meduloblastómu, sú využívané metódy sekvenovania novej generácie a detekcie epigenetických zmien. Cieľom bakalárskej práce je podať ucelený prehľad poznatkov o prístupoch, ktoré boli využité pre určenie molekulárnych podtypov meduloblastómu a o aktuálnych molekulárnych podskupinách v rátane konkrétnych identifikovaných markerov a ich klinickom význame.
