Akutní srdeční selhání a prognóza pacientů po akutním infarktu myokardu s elevacemi ST léčených primární perkutánní koronární intervencí

Acute heart failure and prognosis of the patients with ST elevation myocardial infarction treated with primary PCI

Abstract:

Práce se zabývá prognózou pacientů s akutním infarktem myokardu s elevacemi ST léčených primární perkutánní koronární intervence a zaměřuje se na faktory, které se podílí na rozvoji akutního srdeční selhání (ASS). Celkem bylo vyhodnoceno 593 pacientů, akutní srdeční selhání mělo 27,7% pacientů. Hospitalizační mortalita byla 0,2% ve skupině pacientů bez srdečního selhání a 13% ve skupině pacientů s ASS. Jako faktory nezávisle ovlivňující vznik srdečního selhání a rozvoj dysfunkce levé komory jsme identifikovali zvýšenou glykémie při přijetí, infarkt přední stěny, vyšší věk a tepovou frekvenci, nižší riziko představoval vyšší aortální pulzní tlak a vytvořené kolaterály k infarktové tepně. Provedli jsme genotypizaci inzerčně/delečního (I/D) polymorfismu genu pro ACE, pacienti s genotypem DD/ID měli vyšší end-systolický objem levé komory a horší ejekční frakci. Roční mortalita byla ve sledované skupině pacientů 6,6%, ve skupině pacientů bez ASS 3,0%, ve skupině pacientů s ASS 16,1%.

Abstract:

The aim of the work was to identify the prognosis of the patients after acute myocardial infarction with elevation of ST and to find independent factors with the relationship to acute heart failure (AHF). A total of 593 patients were the study population (27,7% with AHF). The hospitalization mortality was 0,2% within the patients without AHF and 13% within the patients with AHF. Increased blood glucose at admission, myocardial infarction of anterior wall, greater age and higher heart rate were identified as independent factors influencing the development of AHF and left ventricular dysfunction. The lower risk was accounted for higher aortic pulse pressure and created infarct related artery collaterals. Genotyping of I/D polymorhism of ACE gene was performed, higher value of end-systolic volume and lower EF in the DD/ID genotypes group was noted. The one-year mortality was 6,6%, in patients without AHF 3,0%, in patients with AHF 16,1%.
