Applicability of free fetal nucleic acids in mother's peripheral blood for prenatal testing

Abstract:

Mezi nejčastěji využívané přístupy pro neinvazivní prenatální testování plodu patří stanovení volné fetální DNA v krvi matky. Nicméně tato metoda je značně limitována množstvím fetální frakce v krvi matky a jsou proto hledány další možnosti pro monitorování stavu plodu a průběhu těhotenství. Jedním z nejzávažnějších stavů v těhotenství je intrauterinní hypoxie plodu, která může vést k jeho usmrcení či těžkým trvalým následkům u novorozence. V dnešní době nejsou dostupné spolehlivé metody pro diagnostiku hypoxie plodu in utero, které by kromě potvrzení stavu hypoxie plodu určily také její závažnost. Proto je vyvíjeno úsilí pro nalezení nových markerů, které by mohly být využity pro detekci tohoto patologického stavu a umožnily by tak včasné zahájení léčby hypoxie. Mezi nově zkoumané markery patří zejména stanovení změn v expresi genů včetně genů pro microRNA, které jsou spojené s výskytem hypoxie u plodu. Tato práce tedy měla za cíl stanovit míru exprese fetální miR-210 a vybraných protein kódujích genů (HIF-1α, HIF-2α, ADM a LDHA) v krvi matky v době porodu a dále sledovat dynamiku změny této exprese po porodu u matek s hypoxií plodu. Naše výsledky potvrdily expresi fetální miR-210 a testovaných protein-kódujících genů v krvi matky s hypoxií plodu a mohou proto být zvažovány jako markery při neinvazivním vyšetření stavu hypoxie plodu.

Abstract:

One of the most widely used methods for non-invasive prenatal testing is the analysis of circulating cell free fetal DNA in maternal circulation. Unfortunately, this method is greatly limited by the amount of fetal fraction in the maternal blood, and as a result other options for the monitoring of the status of the fetus and monitoring the progress of pregnancy are being researched. One of the most serious conditions during pregnancy is fetal hypoxia, which may result in the death of the fetus or severe permanent childhood disability. At the moment, there are no reliable methods for significant diagnosis fetal hypoxia in utero, which could be used to both confirm the fetal hypoxic status and also determine its severity. Therefore, efforts are being made to find new markers that could be used to detect this pathological condition and enable timely initiation of hypoxia treatment. The recently investigated biomarkers include in particular the gene expression changes, including microRNA genes, which are associated with the presence of fetal hypoxia. Thus this thesis set out to determine the expression of fetal miR-210 and selected protein-coding genes (HIF-1α, HIF-2α, ADM and LDHA) in maternal blood with fetal hypoxia at the time of delivery, and to further monitor the dynamics of expression changes following the labour. Our results confirmed expression of hypoxia-induced miR-210 and selected protein-coding genes in the maternal blood and thus can be considered as a new biomarkers for non-invasive test of fetal hypoxia.
