Sledování autoimunitních fenoménů při protivirové léčbě chronických virových hepatitid

Monitoring of autoimmune phenomena in antiviral treatment of chronic viral hepatitis

Abstract:

Cíl práce: Stanovit výskyt tyreopatií, některých autoprotilátek a parametrů humorální imunity a manifestaci autoimunitních onemocnění u pacientů s chronickými hepatitidami B a C (VHB, VHC) léčených interferonem (IFN). Soubor pacientů a metody: Na klinice infekčního lékařství bylo v letech 1992-2015 léčeno konvenčním nebo pegylovaným IFN 334 pacientů (198 mužů, 136 žen), z nichž 49 mělo VHB a 285 VHC. V průběhu protivirové léčby byly před, během a po léčbě IFN vyšetřeny hladiny thyreostimulačního hormonu (TSH), trijódthyroninu (T3), thyroxinu (T4), protilátky proti štítné žláze: proti thyreoglobulinu (anti-TBG) a mikrosomální frakci štítné žlázy (anti-TPO), protilátky užívané k diagnostice autoimunitní hepatitidy (AIH), a to protilátky antinukleární (ANA), antimitochondriální (AMA), proti hladkému svalu (SMA), proti mikrosomům jater a ledviny (anti-LKM-1), dále protilátky proti dvouvláknové DNA (anti-ds-DNA), proti granulocytům (ANCA), proti deoxy-ribonukleoproteinu (anti-DNP) nebo anti-nukleozómy, revmatoidní faktor (RF) a cirkulující imunokomplexy (CIK). V podskupině 16 pacientů s hemofilií A a 1 pacienta s hemofilií B s VHC byl před, během a po léčbě IFN sledován výskyt inhibitoru faktoru VIII a IX. Současně byla sledována klinická manifestace tyreopatií, autoimunitních onemocnění a u hemofiliků případné krvácivé komplikace. Výsledky: Změny TSH mělo 77 pacientů (24 %), častěji ženy oproti mužům (29 % vs. 19 %), častěji při užití konvenčního než pegylovaného IFN (28 % vs. 18 %). U většiny pacientů mělo onemocnění charakter subklinické hypotyreózy, substituční léčba byla indikována u 32 pacientů. Pouze u 4 pacientů byla diagnostikována a léčena hypertyreóza s klinickým obrazem tyreotoxikózy u jediného pacienta. Autoprotilátky proti štítné žláze byly prokázány u 62 pacientů (19 %), přitom změny TSH se rozvinuly u 30 z nich (48 %), častěji u pacientů s pozitivitou obou autoprotilátek. Nejméně jedna autoprotilátka nebo patologický parametr humorální imunity byly zjištěny u 255 pacientů (76 %), nejčastěji pozitivita ANA a SMA nebo zvýšení RF a CIK. K rozvoji autoimunitního onemocnění došlo u 4 pacientů (1,2 %), jednalo se o AIH, myositidu, myopatii a diabetes mellitus, přitom autoprotilátky měli pouze 3 z nich. V podskupině 17 pacientů s vrozenou koagulopatií byla zjištěna nízká hladina inhibitoru faktoru VIII nebo IX u 6 pacientů s hemofilií A a jediného pacienta s hemofilií B, u žádného pacienta nebyly prokázány častější krvácivé komplikace. Závěr: Dysfunkce štítné žlázy, zejména hypotyreóza, je v průběhu léčby IFN častá, na její rozvoj může upozornit změna TSH nebo přítomnost anti-TBG a anti-TPO, monitorování TSH při léčbě IFN je nezbytné. Výskyt autoprotilátek nebo dalších patologických parametrů humorální imunity je v průběhu léčby IFN častý, ale jejich sledování v běžné praxi není nutné, neboť jen zřídka jsou svázány s rozvojem autoimunitního onemocnění. V podskupině 17 pacientů s hemofilií byly zjištěny jen nízké, klinicky nevýznamné, hladiny inhibitorů koagulačních faktorů, přesto je jejich sledování pro potenciální závažnost plně indikováno.

Abstract:

Aim: To determine the incidence of thyroid diseases, certain autoantibodies and parameters of humoral immunity and the manifestation of autoimmune diseases in patients with chronic hepatitis B and C (VHB, VHC) treated with interferon (IFN). Patients and methods: At the clinic of infectious medicine in the years 1992-2015 a group of 334 patients (198 men, 136 women), of whom 49 had VHB and 285 VHC, was treated with conventional or pegylated IFN. Prior to, during and after completion of IFN treatment, serum levels of thyroid stimulating hormone (TSH), triiodothyronine (T3), thyroxine (T4), anti-thyroid antibodies: anti-thyroglobulin (anti-TBG) and anti-microsomal fraction (anti-TPO); antibodies used for diagnostics of autoimmune hepatitis (AIH): anti-nuclear antibodies (ANA), anti-mitochondrial antibodies (AMA), smooth muscle antibodies (SMA), anti-liver/kidney microsomal antibodies (anti-LKM-1), also antibodies against double-stranded DNA (anti-ds-DNA), anti-granulocytes (ANCA), anti-deoxy-ribonucleoprotein (anti-DNP), or anti-nucleosomes, rheumatoid factor (RF) and circulating immune complexes (CIC) were examined. The inhibitors of factors VIII and IX were investigated in a subgroup of 16 patients with hemophilia A and of 1 patient with hemophilia B with HCV prior to, during and after treatment with IFN. Clinical manifestation of thyroid diseases, autoimmune diseases and potential bleeding complications in hemophiliacs were also monitored. Results. The changes of TSH were observed in 77 patients (24%), more often in women compared with men (29% vs. 19%), more often in patients using conventional rather than pegylated IFN (28% vs. 18%). The disease had the character of subclinical hypothyroidism in the majority of patients; a replacement therapy was indicated in 32 patients. Hyperthyroidism was diagnosed only in 4 patients and was treated, with the clinical picture of thyrotoxicosis in a single patient. Autoantibodies against thyroid gland were detected in 62 patients (19%), while changes in TSH developed in 30 of them (48%), more often in patients with positivity of both autoantibodies. At least one autoantibody or pathological parameter of humoral immunity was detected in 255 patients (76%), most often the positivity of ANA and SMA or an increase of RF and CIC. The development of autoimmune diseases occurred in 4 patients (1,2 %), which were AIH, myositis, myopathy and diabetes mellitus, while autoantibodies were present in only three of them. Low levels of inhibitor of factors VIII or IX were observed in 6 patients with hemophilia A and one patient with hemophilia B from a subgroup of 17 patients with congenital coagulopathies, more frequent bleeding complications were not demonstrated. Conclusion: Thyroid dysfunction, especially hypothyroidism, is in the course of treatment of IFN frequent, its development may highlight the change of TSH or the presence of anti-TBG and anti-TPO, monitoring of TSH is necessary in the treatment of IFN. The occurrence of autoantibodies or other pathological parameters of humoral immunity during IFN treatment is frequent, but their monitoring is not beneficial in routine practice, because they are rarely associated with the development of autoimmune diseases. Only low, clinically insignificant, levels of inhibitors of coagulation factors were observed in a subgroup of 17 patients with hemophilia A and B, however, their monitoring for potential severity is fully indicated.
