The rehabilitative protocol of an elderly patient following a comminuted fracture of the proximal epiphysis of the humerus: clinical case

Abstract:

This thesis describes a particular clinical case, that is a comminuted fracture. More specifically, it is the case of a 69-year-old woman who, falling on cobblestones, incurred in a comminuted fracture of the proximal epiphysis of the left humerus. It will be shown the whole program followed by that patient, from the writing of her personal data to the anamnesis and the short and long-term goals, also talking about inspection, palpation analysis and muscle and joint examinations. In addition, a focused and complete description of the entire treatment plan followed will be made, dividing it over several weeks in order to make more clarity and order within the thesis itself. An essential aspect that has been taken into account is the patient's psychological one, as it was found to be fundamental to the success of the treatment itself. Initially, in fact, the patient was quite demoralized, as she feared losing the job she was doing and burdening too much on the daughter she lived with. For this reason, the objectives and the actual rehabilitative program were clearly presented to the patient, in order to direct her attention to the ultimate goal and not to the momentary problems. The part preceding the clinical case, instead, concerns research topics such as the anatomy of the regions involved in the patient's accident, biomechanics, humerus fractures and recommended treatments. This part of the thesis has a fundamental purpose: giving the necessary information to fully understand the topics covered within the clinical case.

Abstract:

In questa tesi viene illustrato un caso clinico particolare, ossia una frattura pluriframmentaria. Pi? precisamente si tratter? il caso di una signora di 69 anni, la quale, cadendo su dei sanpietrini, si ? provocata una frattura pluriframmentaria dell'epifisi prossimale dell'omero sinistro. Si mostrer? tutto il percorso affrontato con tale paziente, dalla stesura dei dati personali all'anamnesi ed agli obiettivi a breve e lungo termine, parlando anche dell'ispezione, dell'analisi palpatoria e degli esami muscolari ed articolari. Inoltre sar? fatta una descrizione mirata e completa di tutto il piano di trattamento affrontato, suddividendolo nelle varie settimane in modo tale da far pi? chiarezza ed ordine all'interno della tesi stessa. Un aspetto essenziale che ? stato preso in considerazione ? quello psicologico della paziente, in quanto ? risultato fondamentale per il successo del trattamento stesso. Inizialmente, infatti, la paziente era abbastanza demoralizzata, in quanto temeva di perdere il lavoro che svolgeva e di gravare troppo sulla figlia con la quale conviveva. Per tale motivo sono stati esposti in modo chiaro alla paziente gli obiettivi e l'effettivo percorso riabilitativo, in modo da convogliare la sua attenzione sull'obiettivo finale e non sui problemi momentanei. Nella parte precedente al caso clinico, invece, son stati trattati temi di ricerca come l'anatomia delle regioni coinvolte nell'incidente subito dalla paziente, la biomeccanica, le fratture dell'omero ed i trattamenti consigliati. Tale parte della tesi ha uno scopo fondamentale, ovvero quello di dare le informazioni necessarie per comprendere appieno gli argomenti trattati all'interno del caso clinico.
