The nursing process of pregnant woman with gestational diabetes mellitus

Abstract:

Bakalářská práce se zabývá problematikou poruchy metabolismu cukru v těhotenství, tedy tzv. gestačním diabetem. Téma gestační diabetes mellitus je v porodnictví velice aktuální z hlediska narůstající incidence, a proto je pro personál poskytující ošetřovatelskou péči důležité znát všechny informace od projevů až k léčbě a kompenzaci. Bakalářská práce se zaměřuje na ošetřovatelský proces u těhotné ženy s gestačním diabetem v části praktické a teorií nemoci v části teoretické. Cílem této práce je vytvoření ošetřovatelského plánu, který by mohl být využit v praxi porodními asistentkami. V teoretické části práce je popsáno, co je to gestační diabetes a jakým způsobem lze odhalit tuto poruchu u těhotných žen. Dále se zaměřuje na možnosti léčby a kompenzace a v neposlední řadě také na rizika, která z tohoto onemocnění plynou nejen pro matku, ale také pro plod. Na závěr této části je popsána příprava k porodu, průběh samotného porodu a péče o ženu po porodu. V praktické části práce je popsána anamnéza klientky, fyzikální vyšetření provedená porodní asistentkou a hospitalizace klientky v Městské nemocnici Ostrava Fifejdy. Z informací, které byly získány od samotné klientky a od zdravotnického personálu byl sestaven ošetřovatelský proces na základě zpracování posouzení klientky v jednotlivých doménách. Jsou vytvořeny ošetřovatelské diagnózy aktuální i diagnózy potenciální, se kterými se těhotná žena s tímto onemocněním může potýkat. Ošetřovatelské diagnózy jsou sestaveny dle klasifikace NANDA I, taxonomie II. dle konkrétních potřeb ženy s tímto onemocněním.

Abstract:

My bachelor thesis deals with the problem of metabolism of glucose disorder in pregnancy so called gestational diabetes.The topic of gestational diabetes mellitus is very prevalent in obstetrics because of increasing incidence, so it´s really important for nursing stuff to know all the information from symptoms to treatment and compensation. The bachelor thesis focuses on the nursing process of a pregnant woman with gestational diabetes in the practical part. The theoretical part is based on theory of this disorder. The aim of this thesis is to create a nursing process that could be used in practise by other midwives. The theoretical part describes what gestational diabetes is and how to detect this disorder in pregnant woman. It also focuses on the possibilities of treatment and compensation. Last but not least it focuses on the risks, which can rase from this disease not only for women, but also for fetus. At the end of this part there is also a brief summary of some information about preparation for childbirth, birth and nursing care of woman after delivery. Practical part of the thesis describes patient´s anamnesis, physical examination made by a midwife and hospitalization of that client in a Municipal hospital Ostrava Fifejfy. The nursing process, which was assembled mostly from client´s documantation, was based on processing of the client´s assessment of the individual domains. There are obvious diagnoses and potential diagnoses, which pregnant woman can be struggling with. The nursing diagnoses are complied according to NANDA I, taxonomy II. according to specific needs of a woman with this disease.
