Nursing process in pregnant woman with prolonged pregnancy

Abstract:

Bakalářská práce pojednává o problematice prodlouženého těhotenství, se kterým se může na konci těhotenství setkat mnoho těhotných žen. Důležitým prvkem pro určení diagnózy je správná datace termínu porodu a to buď podle prvního dne poslední menstruace, což může být nepřesná metoda kvůli nepravidelnosti menstruačního cyklu ženy, anebo podle ultrazvukové biometrie plodu v prvním trimestru, což představuje přesnější určení gestačního stáří plodu. Ošetřovatelský proces byl sestaven u vybrané klientky, která přišla na porodní sál k plánované indukci porodu kvůli přenášení po termínu porodu. Anamnestické údaje byly sbírány během hospitalizace především formou rozhovoru s klientkou a byly doplněny údaji ze zdravotnické dokumentace a od ošetřujícího personálu. Během hospitalizace bylo u klientky stanoveno 6 aktuálních diagnóz a 3 potencionální. Cíle ošetřovatelské péče byly u většiny diagnóz splněny, kromě diagnózy "Nespavost", kde byl cíl ošetřovatelské péče částečně splněný. Po čtyřdenní hospitalizaci byla žena propuštěna do domácí péče i s dítětem.

Abstract:

The bachelor thesis deals with the issue of prolonged pregnancy, which many pregnant woman may encounter at the end of pregnancy. An important element for diagnosis is the correct date of birth, either according to first day of the last menstruation, which may be an inaccurate method due to woman menstrual cycle irregularity, or according to ultrasound biometrics of fetus in the first trimester, which is more accurate detemination of gestational age. The nursing proces was compiled for a selected client, who came to the labour ward for the planned induction of labour due to the postterm pregnancy. Anamnestic data were collected during hospitalization especially by interviewing the client and were to complete with data from medical documentation and from nursing staff, who took care of the client. During the hospitalization were set for the client six current diagnoses and three potential. The goals of the nursing care have been met at mostly, except the diagnosis insomnisa, which has been met partially. After four days of hospitalization, the woman was discharged to home care with her child.
