Importance and effectiveness of surveillance of first-degree realtives of colorectal cancer patients

Abstract:

Úvod Příbuzní prvního stupně nemocných s kolorektálními neopláziemi mají vyšší riziko vzniku kolorektálního karcinomu. Kolorektální karcinom má hereditární i nehereditární rizika. Cílem naší práce je analýza hereditárních i nehereditárních rizik u příbuzných prvního stupně a identifikace jedinců s nejvyšším rizikem kolorektálního karcinomu, dále porovnání dietních návyků pacientů s kolorektálními neopláziemi a jejich příbuzných prvního stupně, spolu s kontrolní skupinou jedinců bez rodinné anamnézy a bez nálezu neoplastické léze, a nakonec zhodnocení informovanosti pacientů i příbuzných prvního stupně a spokojenosti s kolonoskopickým vyšetřením. Metody Od července 2008 do srpna 2011 probíhal na našem oddělení projekt Jako jedna rodina, kdy jsme sbírali epidemiologická data týkající se kolorektálního karcinomu, informovanosti a spokojenosti s kolonoskopickým vyšetřením od pacientů s kolorektálními neopláziemi, jejich příbuzných prvního stupně a kontrolní skupiny. Příbuzným prvního stupně bylo dle indikačních kritérií provedeno kolonoskopické vyšetření. Vztah rizikových faktorů a přítomnosti neoplastické léze u příbuzných prvního stupně byl popsán logistickou regresí s použitím odds ratio a 95% intervalů spolehlivosti. Ke zhodnocení rizikových faktorů mezi dvěma skupinami byl použit chí kvadrát a Fisherův exaktní test. Výsledky Studijní skupiny tvořilo 242 pacientů s kolorektálními neopláziemi (143 mužů, 99 žen) a 160 přibuzných prvního stupně (66 mužů, 94 žen), z nich 55 s nálezem neoplastické léze (30 mužů, 25 žen), 105 bez nálezu neoplázie (36 mužů, 69 žen). Kontrolní skupinu tvořilo 123 jedinců (66 mužů, 57 žen). Při bivariační statistické analýze byly jako statisticky významné rizikové faktory pro vznik neoplázie u příbuzných prvního stupně zjištěny: mužské pohlaví, věk nad 50 let, sourozenecký vztah s nemocným, kouření a vyšší příjem tuků v potravě. Při následné multivariační analýze vyšel jako nezávislý signifikantní rizikový faktor pouze věk nad 50 let. Většina neoplastických lézí byla nalezena v pravém tračníku. Mezi mužskými i ženskými skupinami pacientů, kontrol a příbuzných s / bez neoplastiké léze srovnávaných podle pohlaví nebylo zjištěno větší množství statisticky významných rozdílů. Ve všech skupinách byly zjištěny statisticky významné rozdíly mezi pohlavími. Závěry V našem souboru potvrzujeme vysokou incidenci neoplastických lézí u příbuzných prvního stupně. K nejrizikovější skupině příbuzných prvního stupně v našem souboru patřili muži, sourozenci, kuřáci, kteří neomezují tuky v potravě. Tato skupina sdílí jak genetické, tak enviromentální rizikové faktory a proto by měla být dispenzarizována s nejvyšší prioritou. Ženy ve všech skupinách mají obecně zdravější dietní návyky než muži. Modifikace screeningových doporučení podle pohlaví by mohla zvýšit efektivitu screeningových programů. Akceptace kolonoskopického vyšetření byla vysoká. Příbuzní prvního stupně jsou informováni o vyšším riziku kolorektálního karcinomu.

Abstract:

Background and aims The first-degree relatives of patients with colorectal neoplasias have higher risk of colorectal cancer than general population. Colorectal cancer has hereditary as well as non-hereditary risks. The aim of our study was to identify first-degree relatives at the highest risk of colorectal neoplasia considering both their hereditary and non-hereditary risks, to compare the dietary habits between colorectal neoplasia patients, their first-degree relatives, and unrelated controls and finaly to evaluate the awareness of patients and their first-degree relatives of risk factors of colorectal cancer and their satisfaction with colonoscopy. Methods The Family project was a unique direct medical counselling project targeting first-degree relatives that took place in our department between July 2008 and April 2011. Within the Project epidemiologic data relevant to colorectal cancer, data about awareness of risk factors and satisfaction with colonoscopy were collected from patients with colorectal neoplasias, their first-degree relatives and control group. First-degree relatives underwent colonoscopic examination if indicated. Odds ratios and 95% confidence intervals were used to describe the associations between risk factors and neoplasia in first-degree relatives. The chi-square or Fisher exact test were used to compare risk factors between two groups. Results The study groups consisted of 242 patients with colorectal neoplasias (143 men, 99 women) and 160 first-degree relatives (66 men, 94 women). Fifty-five (30 men, 25 women) of the first-degree relatives were found to have a neoplastic lesion upon colonoscopy, while the remaining 105 (36 men, 69 women) were without neoplasia. The control group contained 123 individuals with a negative family history for neoplastic lesions (66 men, 57 women). In the bivariate statistical analysis the risk factors for the occurence of neoplastic lesions were: male sex, age over 50 years, sibship, smoking and higher fat intake. In the multivariate analysis only the age over 50 years proved significant. The most of the neoplastic lesions in first-degree relatives were located in the right colon. No significant differences were observed in the dietary habits between the groups of patients, controls and first-degree relatives with / without neoplastic lesions. Nevertheless, statistically significant sex-related differences were observed in all groups. Conclusions We can confirm high prevalence of neoplastic lesions among first-degree relatives. First-degree relatives at the highest risk are men over 50 years of age, siblings, smokers, who do not reduce dietary fat intake. This group of patients share both genetic and environmental risks and thus should be screened with the highest priority. In all groups examined, women had healthier dietary habits than men. Modification of screening guidelines according to sex may improve the efficiency of screening programs. Satisfaction with colonoscopic examination is high. First-degree relatives are aware of their higher risk of colorectal cancer.
