Nursing Care for the Patient with Choroidal Melanoma Requiring Enucleation

Abstract:

Bakalářská práce se zabývá problematikou ošetřovatelské péče o pacienty s melanomem cévnatky vyžadujícím enukleaci. Cílem práce bylo zmapovat řešení problematiky melanomu cévnatky vyžadující enukleací očního bulbu u vybraných pacientů Oční kliniky Fakultní nemocnice Brno, se zaměřením na oblast ošetřovatelské péče a edukace těchto pacientů, popř. podat návrhy na změnu v přístupech v uvedených oblastech, které by měly vést k vyšší úrovni zajištění zmíněné péče a edukace. Řešení práce je postaveno na kvalitativním výzkumu prostřednictvím zpracovaných kazuistik. Ke sběru informací byly využity techniky analýzy dokumentů, přímé pozorování, polostrukturované rozhovory s pěti klienty/pacienty podstupujícími chirurgický zákrok - enukleaci, z důvodu melanomu cévnatky. Z kazuistik vyplynulo, že na vybraném zdravotnickém pracovišti je prováděná perioperační ošetřovatelské péče a edukace na dobré úrovni. Kazuistiky nám však také pomohly nalézt oblasti, kde je možné ošetřovatelskou péči a edukaci pacientů po enukleaci očního bulbu doplnit a zdokonalit. Na tyto oblasti je zaměřen návrh do praxe (mapa péče u pacientů před enukleací a po enukleaci a edukační materiály: edukační záznam, edukační plán, edukační leták na téma: "Průvodce pacienta péčí o oční důlek" aj.).

Abstract:

The bachelor's thesis deals with nursing care for patients who suffer from uveal melanoma and need to undergo enucleation. The aim of the work was to map the solution of the issue of choroidal melanoma requiring enucleation in selected patients of the Eye Clinic of the University Hospital Brno, focusing on the field of nursing care and education of these patients, or make proposals for a change in approaches in these areas, which should lead to a higher level of provision of the mentioned care and education. The solution of the work is based on qualitative research using the case history method. Techniques of document analysis, direct observation, and semi-structured interviews with five clients/patients undergoing surgery - enucleation, due to uveal melanoma, were used to gather information. The case reports has shown that perioperative nursing care and education are performed at a good level at the selected health care facility. Furthermore, the case reports also helped us to find areas where it is possible to supplement and improve nursing care and education of patients after eyeball enucleation. The proposal for practice is focused on these areas (map of care for patients before and after enucleation and educational materials: educational record, educational plan, educational leaflet on the topic: "A Patient´s Guide to Care of the Eye Socket", etc.).
