Monitoring of selected aminothiols in patients with a high risk of atherosclerosis

Abstract:

Homocystein a některé další aminothioly (cystein a cysteinyl-glycin) by mohly být považovány za rizikové faktory při rozvoji aterosklerózy a následných kardiovaskulárních onemocnění. U těchto látek se zjistila řada negativních účinků na lidský organismus - mohou poškozovat cévní endotel, ovlivňovat koagulační rovnováhu či podporovat produkci volných radikálů. Zaměřili jsme se na některé skupiny pacientů s onemocněními, která představují vyšší riziko pro rozvoj aterosklerózy a stanovovali u nich hladiny aminothiolů pomocí metody vysoce účinné kapalinové chromatografie s elektrochemickou detekcí. Hladiny celkového cysteinu, homocysteinu, cysteinyl-glycinu a methioninu jsme proměřili u diabetiků, hemodialyzovaných a obézních pacientů a porovnávali je s hodnotami v jejich kontrolních skupinách. U diabetických pacientů jsme naměřili významně zvýšené hladiny celkového cysteinu, homocysteinu a cysteinyl-glycinu, u hemodialyzovaných pacientů zvýšené hodnoty cysteinu a homocysteinu a u obézních pacientů hladiny cysteinu. Pozitivní korelaci mezi homocysteinem a cysteinem jsme zaznamenali při hladině homocysteinu do 20 ?mol/l u diabetiků a obézních pacientů. Lipoprotein(a) je považován za rizikový faktor pro rozvoj aterosklerózy. Jedná se o tzv. pojítko mezi aterosklerózou a trombózou. Vzhledem ke struktuře lipoproteinu(a) a volné thiolové skupině u aminothiolů se nabízí možnost, že aminothioly potencují patologické účinky lipoproteinu(a). Hladiny lipoproteinu(a) byly významně zvýšené jen u obézních pacientů. Skupiny se významně nelišily co do prevalence současně zvýšených hodnot lipoproteinu(a) a cysteinu, lipoproteinu(a) a homocysteinu. Pokusili jsme se modifikovat metodu pro stanovení redukovaných a oxidovaných aminothiolů, jejichž hladiny se pohybují v krvi ve velmi nízkých koncentracích. Vzhledem k výskytu řady interferujících látek je jejich stanovení touto metodou obtížné. Metoda je vhodná pro stanovení celkových a volných aminothiolů. Pro stanovení redukovaných aminothiolů se zdá jednodušší zvolit jinou formu detekce.

Abstract:

Homocysteine and other aminothiols (cysteine and cysteinylglycine) could be considered as a possible risk factors in the development of atherosclerosis and cardiovascular diseases. It was discovered that these substances have many negative effects on the human organism. They can impair endothelial cell, they can have an influence on the coagulating balance or they can help to produce free radicals. We focused on some patients with the risk to the development of atherosclerosis. We determined their levels of aminothiols by the high-performance liquid chromatographic method in these groups of patients. We measured the levels of total cysteine, homocysteine, cysteine-glycine and methionine in diabetic, hemodialysis and obese patients and compare them with the controls groups. In diabetic patients, we found out significantly elevated levels of total cysteine, homocysteine and cysteinyl-glycine, in hemodialysis patients there were elevated levels of cysteine and homocysteine and in obese patients were elevated levels of cysteine. Significant correlations were found between homocysteine and cysteine in samples with homocysteine ? 20 ?mol/l in diabetic and obese patients. Lipoprotein(a) is considered as a risk factor for atherosclerosis. Lipoprotein(a) may represent a link between the fields of atherosclerosis and thrombosis. Regarding the structure lipoprotein(a) and free sulfhydryl group of aminothiols is there the potential synergism between them. Aminothiols can increase the reactivity of the lipoprotein(a). Levels lipoprotein(a) were significantly elevated only in obese patients. There were no significant difference in groups between prevalence of parallel elevated levels lipoprotein(a) and cysteine, lipoprotein(a) and homocysteine. We tried to modify the method for measurement of reduced aminothiols. Levels of reduced aminothiols are in plasma in a very low concentration. Regarding the appearance of interferences substances is their determination with this method difficult. The method is convenient to determine of total and free aminothiols. We suggest other forms of detection for determination of reduced aminothiols.
