Abstract:

Úvod: Tématem mé bakalářské práce je fyzioterapie v oblasti powerliftingu a zranění tímto sportem způsobená. Seznámením s tímto tématem bych chtěla poukázat na náročnost této sportovní disciplíny a možnosti fyzioterapeutické intervence jak za účelem léčby zranění, tak pro prevenci jejich vzniku. Cíl práce: Cílem práce je přispět k řešení problematiky fyzioterapie u zranění způsobených powerliftingem, konkrétněji pak fyzioterapie u impingement syndromu ramenního kloubu způsobeného dlouhodobým přetěžováním v důsledku tohoto sportu. Metodika: Hlavní část mé bakalářské práce obsahuje kazuistiku mladého profesionálního powerliftera s diagnostikovaným impingement syndromem ramenního kloubu. Naše spolupráce trvala 5 týdnů, ve kterých proběhlo 10 terapií. Před zahájením fyzioterapeutické intervence jsem provedla vstupní kineziologické vyšetření (vyšetření aspekcí, palpací, goniometrie a antropometrie horních končetin, vyšetření pomocí speciálních testů na impingement syndrom, vyšetření zkrácených struktur a pohybových stereotypů). Na základě výsledků tohoto vyšetření jsem vypracovala krátkodobý rehabilitační plán pro následujících pět týdnů terapie. Náplní jednotlivých terapií bylo využití fyzioterapeutických konceptů a metod (Měkké a mobilizační techniky, PIR, BPP, DNS, Kinesio taping, Brügger koncept, PNF a jiné) k ošetření měkkých tkání, mobilizaci kloubních blokád, stabilizaci lopatky, zlepšení rozsahu a svalové síly v oblasti ramenního kloubu a nácviku správného stereotypu pohybu. Před ukončením terapie jsem provedla výstupní kineziologický rozbor, z jehož výsledků jsem vycházela při stanovení dlouhodobého fyzioterapeutického plánu. Výsledky: Zaznamenala jsem viditelné zlepšení v aktivním rozsahu pohybu a svalové síly do abdukce a vnitřní rotace v ramenním kloubu (abdukce z 80 ° na 150 °, vnitřní rotace ze 40 ° na 60 °, svalová síla do abdukce z 3. na 4. stupeň svalového testu). Nejvýraznější změna nastala pro pacienta ve snížení bolestivosti při pohybu do abdukce a vymizení klidových a nočních bolestí. Závěry: Závěrem mé práce jsem zhodnotila výsledky vstupního a výstupního kineziologického vyšetření. Fyzioterapeutická intervence měla pozitivní vliv na život pacienta a z dlouhodobého hlediska i na kvalitu provedení jeho sportovních výkonů.

Abstract:

Introduction: The topic of this bachelor thesis is physiotherapy in the area of powerlifting and injuries caused by this sport. The objective: The goal of this thesis is to contribute and to solve issue concerning physiotherapy by the right choice of physiotherapeutic concepts and methods in patients suffering a powerlifting based injury with the main focus on shoulder impingement syndrome. Methodology: The main part of the thesis contains a case report of the young professional powerlifter with the shoulder impingement syndrome. The patient was provided with care for 5 weeks, which contained 10 sessions. Before the physiotherapeutic intervention I had performed a kinesiological examination (aspection and palpation examination, goniometry and anthropometry of upper limbs, impingement syndrome special tests, examination of shortened structures and stereotypical movements). On the basis of the results I prepared a short-term rehabilitation plan. Each therapeutic session consisted of specific physiotherapeutic concepts and methods such as: Soft tissue techniques, Mobilisations, PIR, BPP, DNS, Kinesio taping, Brügger koncept, PNF etc. All these methods helped to treat soft tissues, to restore the joint play and scapula stabilization, to improve range of motion and muscle strength in the shoulder joint and to provide the correct stereotype of movement. Before the end of the treatment I performed the final kinesiological examination and created a long-term rehabilitation plan based on the results. Results: I noticed a significant improvement in the active range of motion and the muscle strength in abduction and inner rotation of the shoulder joint (abduction 80 150 deg., inner rotation 40 60 deg., muscle strength in abduction from third to fourth degree of the muscle strength test). The most obvious change for the patient was the reduction of the pain while abduction and the disappearance of night and still pain. Conclusions: At the end of the thesis I evaluated the results of entry and final kinesiological examination. This physiotherapeutic intervention had a positive effect on the quality of the patient's life and in the long term also of his sport performances.
