Kinematic Gait Analysis of Pregnant Women

Abstract:

Těhotenství je náročným obdobím v životě ženy. Během těhotenství dochází v těle těhotné ženy k řadě změn, které umožňují růst a vývoj budoucího dítěte a jeho následný porod. Tyto změny se odrážejí v rozsahu pohybu trupu, pánve, dolních končetin a dalších částí těla, ovlivňují lokomoci a některé z těchto změn mohou přetrvávat i v poporodním období. Cílem této práce bylo popsat změny ovlivňující chůzi během těhotenství a určit vliv obuvi J Hanák R na kinematiku chůze v průběhu těhotenství a v poporodním období. Předchozí studie naznačují, že speciální ortopedické stélky a obuv by mohly sloužit jako prevence bolesti a diskomfortu pohybového aparátu, častých v průběhu těhotenství. Longitudinální design této studie se skládal ze tří měření během těhotenství, a to ve 14., 28. a 37. gestačním týdnu, a jednoho měření 28 týdnů po porodu. Tento design umožňoval zachytit kompletní změny v chůzi v průběhu těhotenství v kontrolní skupině (n=18), bez intervence, a v experimentální skupině (n=23), která používala testovanou obuv. K zachycení změn rozsahu pohybu v různých segmentech těla byla použita kinematická analýza. Rozdíly mezi kontrolní a experimentální skupinou při prvním měření pozorované u většiny analyzovaných proměnných, stejně jako jejich relativně vysoké směrodatné odchylky naznačují velké individuální rozdíly v provedení chůze, které znesnadňují hodnocení testované obuvi. Vliv testované obuvi na kinematiku chůze může být vysvětlen skrze preventivní účinek této obuvi proti propadu nožní klenby. Na rozdíl od kontrolní skupiny, v experimentální skupině bylo zjištěno statisticky významné snížení v zakřivení páteře, zvýšená maximální flexe v kyčelním kloubu, snížený čas krokového cyklu a nárůst ve výšce kolenního kloubu 28 týdnů po porodu ve srovnání s těhotenstvím. V kontrolní skupině byly během pokročilých fází těhotenství a po porodu pozorovány změny spojené v předchozích studiích s propadem nožní klenby a hyperpronací paty, např. zvýšená flexe v kolenním kloubu nebo výraznější zakřivení páteře. Pro kompletní zhodnocení testované obuvi na vzorec chůze během těhotenství jsou však zapotřebí budoucí studie kombinující kinematické a dynamické metody analýzy chůze.

Abstract:

Pregnancy is a very demanding period of woman's life. During pregnancy, an array of changes occurs in her body to enable the growth and development of the future baby and the consequent delivery. These changes are reflected in the range of motion of trunk, pelvis, lower limbs and other body segments, affect the locomotion and some of these changes may persist to the postpartum period. The aim of this thesis was to describe the changes affecting the gait during pregnancy and to determine the effect of footwear J Hanák R on kinematic gait characteristics during pregnancy and in postpartum period as previous studies indicate that special orthopaedic insoles and footwear might be useful in prevention of the common musculoskeletal pain and discomfort related to pregnancy. The longitudinal design of this study consisted of three measurements during pregnancy period, at the 14, 28 and 37 gestational weeks and one 28 weeks postpartum to capture the complete pregnancy-related changes in gait in the control group (n=18), without any intervention, and in the experimental group (n=23), which was wearing the tested shoes. Kinematic analysis was used to capture the changes in range of motion in different body segments. The differences between the control and experimental group at the first data collection session in most of the analysed variables, as well as relatively high standard deviations of analysed variables indicate large individual differences in the gait pattern making the evaluation of tested footwear difficult. The effect of tested footwear on kinematic gait pattern changes may be explained by its preventive effect against the foot arches falling. Unlike the control group, in the experimental group statistically significant spinal curvature straitening, increased maximal flexion in hip joint, decreased step cycle time and increase in knee height were observed postpartum compared to pregnancy. In the control group, changes associated previously with the foot arches falling and hindfoot hyperpronation were observed during advanced phases of pregnancy and postpartum, e.g. increase in knee flexion or increase in spinal curvature. However, for the comprehensive evaluation of the tested footwear on pregnancy gait pattern, future studies combining the kinematic and dynamic plantographic methods are needed.
