Effect of neuromuscular activation of the hip joint abductor muscles on the foot position

Abstract:

Inhibice aktivity abduktorů kyčelního kloubu nebo pronační postavení nohy můžou mít vliv na správnou funkci dolní končetiny a páteře. Dle studií existuje spojitost mezi sníženou neuromuskulární aktivitou abduktorů kyčelního kloubu a postavením nohy. Cílem práce je zjistit, jaký vliv má cvičení v systému Redcord zaměřené na svaly kyčelního kloubu na pronační postavení nohy. První část práce je věnována teoretickým poznatkům o anatomii a kineziologii kyčelního kloubu a nohy, neuromuskulární kontrole pohybu a také pohledu na myofasciální zřetězení ovlivňující právě kyčelní kloub i klouby nohy. Výzkum je zaměřen na vliv cvičební pozice Side-Lying Hip Abduction v systému Redcord na postavení nohy. Výzkumný soubor byl složen z 6 mladých závodníků v horské cyklistice (průměrný věk 18,83 ? 0,9 let, průměrná výška 183,5 ? 4,2 cm, průměrná hmotnost 73,0 ? 5,86 kg). Cyklisti často trpí bolestmi zad a mají sklony k patelofemorálnímu syndromu, které mohou být následkem právě pronačního postavení nohy a snížené aktivity abduktorů kyčelního kloubu. Také postavení dolních končetin při tzv. úzkém šlapání na kole vede k přetížení adduktorů kyčelního kloubu a nízkému zapojení abduktorů. Pro zhodnocení aktivity abduktorů kyčelního kloubu bylo využito měření akčního potenciálu pomocí elektromyografie během stoje na jedné dolní končetině. Pro hodnocení postavení nohy byl využit Navicular Drop Test. Terapeutická intervence se skládala z 8 cvičebních jednotek, které byly zaměřeny pouze na aktivace abduktorů kyčelního kloubu v systému Redcord. Staticky významné změny se ukázaly pouze u aktivity levostranného musculi glutei medii (p = 0,046) a Navicular Drop Testu pro levou dolní končetinu (p = 0,03), ostatní změny se ukázaly jako statisticky nevýznamné.

Abstract:

Inhibition of the hip abductors activity or the pronation of the foot may affect the proper function of the lower limb and spine. According to studies, there is an association between decreased neuromuscular activity of hip abductors and foot position. The aim of the thesis is to influence the position of the foot by activating these muscles, in the sense of reducing the pronation. The first part of the thesis is devoted to theoretical knowledge about the anatomy and kinesiology of the hip joint and foot, neuromuscular control of movement and myofascial chains affecting the hip joint and leg joints. The research is focused on the influence of the Side-Lying Hip Abduction exercise position in the Redcord on the foot position. The research group consisted of 6 young mountain bikers (average age 18.83 ? 0.9 years, average height 183.5 ? 4.2 cm, average weight 73.0 ? 5.86 kg). Bikers often suffer from back pain and a tendency to patellofemoral syndrome, which may be due to the pronation position of the foot and reduced activity of the abductors of the hip joint. Also, the position of the lower limbs when pedaling the bike leads to overloading of the hip joint adductors and low engagement of the hip abductors. We used electromyography to evaluate the activity of hip abductors during one leg stand. The Navicular Drop Test was used to evaluate the position of the foot. The therapeutic intervention consisted of 8 exercise units, which were focused only on the activation of hip abductors in the Redcord. Statically significant changes were shown only in the activity of the left gluteus medius muscle (p = 0.046) and the Navicular Drop Test for the left lower limb (p = 0.03), the other changes proved to be statistically insignificant.
