In-Hospital Cardiac Arrest as viewed by a Medical Emergency Team

Abstract:

Bakalářská práce se zaměřuje na problematiku kardiopulmonální resuscitace ve zdravotnickém zařízení Krajské zdravotní a.s. - Masarykovy nemocnice o.z. v Ústí nad Labem. Mapuje problematiku a specifika poskytování resuscitační péče ve zdravotnickém zařízení spolu se současnou situací v oblasti kardiopulmonální resuscitace. Teoretická část se zabývá problematikou poskytování kardiopulmonální resuscitace v obecné rovině, popisuje život ohrožující závažné stavy, definuje defibrilaci jako důležitou lékařskou metodu. Velkou pozornost věnuje kardiopulmonární resuscitaci v nemocničním prostředí a popisuje práci resuscitačního týmu jako specializované složky, která je nezbytná pro zajištění vysoce specializované péče v nemocničním zařízení. Praktická část této práce formuluje problém, cíle a metodologii průzkumného šetření, které jsou zaměřeny na problematiku poskytování kardiopulmonální resuscitace v praxi. Hlavní cíl je orientován na sledování a hodnocení celkové úspěšnosti kardiopulmonální resuscitace u pacientů s náhlou zástavou oběhu. Dílčí cíle poté zjišťují hodnocení časové dostupnosti resuscitačního týmu spolu s hodnocením připravenosti zdravotníků u pacienta s náhle vzniklým zhoršením stavu do doby před příchodem resuscitačního týmu. Současně také sledují nejčastější dobu výskytu náhlé zástavy oběhu v kontextu pohlaví. Výsledky jsou následně přehledně rozebrány a zpracovány. Získané výsledky budou předloženy managementu zdravotnického zařízení spolu s doporučeními, které budou vycházet i z nejnovějších trendů v oblasti neodkladné kardiopulmonální resuscitace, včetně návrhu karty včasného varování pro lůžkové oddělení i ambulantní části.

Abstract:

The bachelor thesis focuses on the issue of in-hospital cardiac arrest (IHCA) in the medical facility Krajské zdravotní a.s. - Masaryk Hospital o.z. in Ústí nad Labem. It maps the issues and specifics of the provision of resuscitation care in a medical facility together with the current situation in the field of cardiopulmonary resuscitation. The theoretical part deals with the issue of providing general cardiopulmonary resuscitation, describes life-threatening serious conditions, defines defibrillation as an important medical method. We pays great attention of IHCA and describes the work of the medical emergency team as a specialized component, which is necessary to provide highly specialized care in a hospital facility. The practical part of this work formulates the problem, goals and methodology of the survey, which is focused on the issue of providing cardiopulmonary resuscitation in practice. The main goal is to monitor and evaluate the overall success of IHCA with return of spontaneous circulation (ROSC). The sub-objectives then set the evaluation of the time availability of the medical emergency team together with the evaluation of the readiness of the medical staff for the patient with a sudden deterioration of the condition until the arrival of the medical emergency team. At the same time, they follow the most common period of IHCA the context of gender. The results will be clearly analyzed and processed. The obtained results will be presented to the management of the medical facility together with recommendations that will be based on the latest trends in resuscitation care, including the design of an Early Warning Card (EWS) for the inpatient department and the outpatient department.
