Characteristics of microbial agents demonstrated in urine specimens from the urological ambulance and the general practitioner

Abstract:

Bakalářská práce pojednává o bakteriálních původcích močových infekcí, zejména o gramnegativních bakteriích, které jsou v dnešní populaci nejčastější příčinou zánětu močového ústrojí. Teoretická část bakalářské práce je věnována bližšímu popisu močových infekcí, klasifikaci na akutní a chronické záněty urogenitálního traktu a odborné charakteristice nejčastějších a také méně se vyskytujících bakteriálních původců močových infekcí. Následně jsou uvedeny metody identifikace a také přístup k terapii v závislosti na původci onemocnění. Praktická část je stručně zaměřena na zpracování vzorku moče po přijetí do laboratoře, postupu kultivace na běžných a selektivních diagnostických půdách, testovaní citlivosti k antibiotikům a použití hmotnostní spektrometrie při identifikaci bakterií. Na základě získaných výsledků takto provedeného vyšetření porovnám četnost výskytu jednotlivých kmenů, citlivost kmenů k testovaným antibiotikům, počet infekcí v závislosti na pohlaví a také věku, nejčastější typ urologických diagnóz a počet ESBL+ kmenů. Srovnání výsledků je provedeno vyhodnocením 150 vzorků ze 7 ambulancí všeobecných lékařů, a to v období od měsíce března do září roku 2017 a 150 vzorků ze 4 ambulancí urologů, v období měsíce září roku 2017. K získávání dat je využíván laboratorní informační systém ENVIS.

Abstract:

Bachelor thesis deals with bacterial agents of urinary infections, especially gramnegative bacteria, which are the most frequent cause of urinary tract inflammation in today's population. The theoretical part of the bachelor thesis is devoted to a more detailed description of urinary infections, classification for acute and chronic inflammation of the urogenital tract, and professional characteristics of the most frequent and also less occurring bacterial agents of urinary infections. Subsequently identification methods are listed as well as access to therapy depend on the origin of the disease. The practical part is briefly focused on the processing of urine samples after admission to the laboratory, the cultivation procedure on conventional and selective diagnostic soils, antibiotic sensitivity testing and the use of mass spectrometry for bacterial identification. On the basis of the results of the examinations carried out, I compare the occurrence of individual strains, the susceptibility of the strains to the tested antibiotics, the number of infections by gender and also the age, the most common type of urological diagnosis and the number of ESBL+ strains. A comparison of the results is done by evaluating 150 samples from 7 ambulances of general practitioners from March to September 2017, and samples from 4 urology ambulances in September 2017. The ENVIS laboratory information system is used to obtain data.
