Multidisciplinary Co-operation at the Home Care of Long-Term Sick Seniors

Abstract:

Diplomová práce se zaměřuje na problematiku multidisciplinární spolupráce v rámci domácí péče u dlouhodobě nemocných seniorů. Hlavním cílem práce je zjistit, jak sociální pracovníci vnímají multidisciplinární spolupráci v rámci domácí péče a zdali využívají i některé z nástrojů multidisciplinární spolupráce. Diplomová práce je členěna do části teoretické a praktické. Teoretická část se zabývá multidisciplinární spoluprací, specifiky domácí péče u seniorů a krátkým nástinem dlouhodobých onemocnění u seniorské populace. Zásadní částí diplomové práce je kvalitativní výzkum, který navazuje na teoretické poznatky a prostřednictvím kterého je dosaženo výzkumného cíle, zanalyzovat současnou úroveň, potřebnost, bariéry a limity multidisciplinární spolupráce v rámci domácí péče z pohledu sociálních pracovníků.

Abstract:

The diploma thesis focuses on the issue of multidisciplinary co-operation in home care for long-term sickness seniors. The main objective of the work is to find out how social workers perceive multidisciplinary co-operation in home care and whether they also use some of the tools of multidisciplinary cooperation. The thesis is divided into the theoretical and practical part. The theoretical part deals with multidisciplinary cooperation, specifics of home care in the elderly and a short outline of long-term illnesses in the senior population. The fundamental part of the thesis is qualitative research, which builds on the theoretical knowledge and through which the research goal is achieved, analyze the current level, need, barriers and limits of multidisciplinary cooperation within home care from the perspective of social workers.
