Hearing impairment after the cisplatin chemotherapy in children

Abstract:

V předložené práci bylo prokázáno největší poškození sluchu u pacientů s meduloblastomy konkrétně středního rizika (protokol CCGA 9961) a u pacientů s neuroblastomy. (ANBL 02P1). U těchto skupin dochází ke zhoršování sluchu i řadu let po ukončení chemoterapie. Jedná se především o malé děti, do 5 let, které jsou k účinkům cisplatiny nejsenzitivnější. Naproti tomu pacienti s osteosarkomy a germinálními tumory, i přes vyšší kumulativní dávku cisplatiny užité k chemoterapii, jsou, s největší pravděpodobností pro svůj vyšší věk, postiženi ztrátou sluchu méně a sluch se jim s odstupem let prakticky nemění. Se vzrůstající kumulativní dávkou cisplatiny nedochází k výraznějšímu zhoršování sluchu. Věk jako rizikový faktor zhoršení sluchu byl prokázán klinicky i statisticky. Nejsenzitivnější jsou děti do 5 roků věku. Pacienti, u nichž během terapie došlo k úbytku váhy, měli na konci chemoterapie horší sluch než pacienti, kteří byli váhově stabilní. Tato závislost nebyla statisticky prokázána.

Abstract:

In the submitted work a greater hearing loss was proven in patients with medulloblastomas, specifically of medium risk (protocol CCGA 9961) and for patients with neuroblastomas (ANBL 02P1). For these groups there is also a deterioration of hearing many years after the end of chemotherapy. This primarily involves small children up to the age of 5, who are the most sensitive to the effects of cisplatin. Compared with this patients with osteosarcomas and germinal tumours are less impacted by hearing loss, in spite of the higher cumulative dose of cisplatin used for chemotherapy, in all probability due to their higher age, and years later their hearing practically does not change. There is no more significant hearing loss with increasing cumulative dose of cisplatin. Age as a risk factor for hearing loss was proven clinically and statistically. Children up to the age of 5 are the most sensitive. Patients who lost weight during therapy had worse hearing at the end of chemotherapy than patients whose weight remained stable. This linkage was not proven statistically.
