The Relationship between the Level of Cognitive Function and Self-Sufficienty of Elderly People

Abstract:

Cíl: Cílem práce bylo stanovit, zda úroveň kognitivních funkcí u seniorů ovlivňuje jejich míru soběstačnosti. Zjistit a statisticky porovnat, zda je kognice a soběstačnost závislá na pohlaví, věku a dochází ke změnám v čase. Metodika: Výzkumný soubor tvořilo 72 seniorů ve věku 65 roků a více, kteří byli uživateli sociálních služeb v Domě sociálních služeb v Ostravě-Radvanicích (prosinec 2011 ? červenec 2012). Dotazníkové šetření bylo provedeno pomocí standardizovaných dotazníků. Poruchy kognice byly testovány dotazníkem Mini Mental State Examination ? Folsteinův test, ke zjištění míry soběstačnosti byl použit test ADL ? Test základních všedních činností podle Barthelové. Dotazníky byly vyhodnoceny standardizovaným hodnotícím nástrojem: Kruskal-Wallisův neparametrický test pro více výběrů, Fisherův exaktní test, dvouvýběrový t-test, neparametrický párový Wilcoxnův test. Dále bylo využito Spearmanovy korelační analýzy (rs) pro vztah MMSE a ADL. Statistické výsledky byly hodnoceny na hladině významnosti 5 %. Výsledky: Naše studie potvrdila významný vztah mezi dotazníky MMSE a ADL v prvním (rs=0,5927, p=0,001), druhém (rs=0,6870, p=0,001) i třetím (rs=0,7006, p=0,001) měření. Šetřením bylo zjištěno, že míra kognitivních funkcí ovlivňuje soběstačnost seniorů. Zároveň byly zjištěny významné nižší hodnoty u testu ADL ve 3. šetření (p=0,0127) , proti hodnotám v 1. (p= 0,6735) a 2. (p=0.0415) šetření. Zároveň byl zjištěn u prvního měření ADL a porovnání věkových skupin významný statistický rozdíl (p= 0,033), kdy u vyšší věkové skupiny byla zaznamenána menší samostatnost při koupání, při opakovaném měření se však toto tvrzení neprokázalo. Závěr: Součástí práce je návrh opatření pro uchování co nejvyšší míry soběstačnosti, která nejvíce určuje autonomii v rozhodování seniorů, v jejich aktivitě a nejvyšší možné kvalitě života. Zároveň podpora zachování míry kognitivních funkcí má vliv na soběstačnost a rozhodování o způsobu prožití života.

Abstract:

Object: The objective of thesis was to determine whether the level of cognitive functions in the seniors affects their degree of self-sufficiency. To find out and statistically compare whether cognition and self-sufficiency is dependent on sex, age and there are changes over time. Methodology: The research group was consisted of 72 seniors aged 65 years and over who were users of social services in the House of social services in Ostrava-Radvanice (December 2011 - July 2012). The questionnaire survey was carried out using standardized questionnaires. Disorders of cognition were tested questionnaire Mini Mental State Examination - Folstein Test, to determine the degree of self-sufficiency was used ADL - Activities of daily living Test by Barthel. The questionnaires were evaluated by a standardized assessment tool: Kruskal-Wallis nonparametric multiple-choice test, Fisher's exact test, two-choice t-test, nonparametric paired Wilcoxn Test. It was also used Spearman correlation analysis (rs) for the relationship between MMSE and ADL. Statistical results were evaluated at a significance level of 5%. Results: Our study confirmed a significant relationship between MMSE and ADL questionnaires in the first (rs = 0,5927, p = 0,001), second (rs = 0,6870, p = 0,001) and third (rs = 0,7006, p = 0,001) measurement .The research found that the degree of cognitive functions affects the self-sufficiency of seniors. They were also found significant lower values in the 3rd survey of Test ADL (p = 0,0127) compared to the values in the 1st (p = 0,6735) and the 2nd (p = 0,0415) survey. It was also discovered in the first ADL measurement and comparison of age groups significant statistical difference (p = 0,033), where in the higher age group was detected less independence when bathing, however repeated measurement did not prove it. Conclusion: Part of thesis is proposal of measures to keep the highest possible degree of self-sufficiency that most determines autonomy in senior decision-making, in their activity and the highest possible quality of life. At the same time preserving of cognitive functions rate has an effect on self-sufficiency and decisions about how to spend their life
