Community midwife care during pregnancy and childbirth

Abstract:

Cíl: Prvním výzkumným cílem bylo zjistit, jak se ženy cítí v těhotenství, co prožívají, jaká jsou jejich očekávání. Druhým výzkumným cílem bylo zjistit, co ženy zajímá v předporodním kurzu a vytvořit edukační program pro jimi stanovené téma. Třetím výzkumným cílem bylo zjistit, jak ženy prožívají porod, jaké jsou jejich první pocity po porodu, zda jim rady z předporodního kurzu byly užitečné. Metodika: Výzkumný soubor tvořilo 6 skupin po čtyřech klientkách, které v období říjen 2010 až leden 2011 navštěvovaly kurz předporodní přípravy v Chebské nemocnici. Ze skupin byly vybrány 3 respondentky, které se zúčastnily rozhovoru a pozorování. Kritériem pro výběr byla návštěvnost všech lekcí předporodního kurzu, souhlas s rozhovorem a následný porod v Chebské porodnici. Výsledky: Dle polostrukturovaných rozhovorů bylo zjištěno, že dotazované ženy měly z těhotenství dobrý pocit a těšily se z něj. V jednom případu však dotazovaná uvedla, že se potýkala s problémy a další těhotenství neplánuje. Při vytváření edukačních plánů jsem se inspirovala návrhy samotných respondentek. Tyto edukační plány jsou používány nadále v kurzech předporodní přípravy a plní svůj účel při edukaci dalších klientek. Při přítomnosti u porodu jsem vykonávala svou práci porodní asistentky a komunikovala a pozorovala ženu během porodního děje. Následným dotazováním po porodu bylo zjištěno, že nejvíce bylo využito dýchacích technik při porodu a úlevových poloh. Pocity, které ženy prožívaly, byly většinou úleva a radost. Závěr: Porodní asistentka je důležitou osobou nejen pro těhotnou a rodící ženu. Díky svému vzdělání, zkušenostem a radám by měla být schopna poradit ženě v jakékoli oblasti týkající se těhotenství, porodu, šestinedělí a péči o dítě. Dle dnešních možností má žena na výběr, jak se na porod připravit a je na jejím vlastním uvážení, kde bude porod probíhat.

Abstract:

Objective: The first research target of this thesis was to find out, how do the women feel in the pregnancy, what do they live through, which expectations do they have. The second target was to find out, what about are the women interested in the antenatal course at most and to create out an education program for them. The third target was to find out, how do the women live through the childbirth, which are their first feelings after the childbirth and if the antenatal coaching was useful for them. Methodology: The experimental set was built of 6 groups with 4 clients in each group. They had attended the antenatal course before childbirth in the hospital in Cheb from October 2010 to January 2011. From groups were chosen 3 clients, who took part in an interview and an observation.The criterion for the choose was the number of attendance at the antenatal courses, the agreement with the interview and the following childbirth in the Cheb´s hospital. Results: On the basis of the semi-structured interviews was find out, that the women had a very good feeling from the pregnancy and they were happy about it. But in one case one interviewed woman reffered she had fought with problems and she is not planning anotherpregnancy. By the creating of the education plans I have been inspired with the own clients´ ideas. These education plans are used by the antenatal courses and they fill thier purpose by education another clients. By my present at the childbirth I did the work of the midwife, I have also communicated with a woman and observated her during the childbirth. With the following interview was find out, that by the childbirth were used the breathing techniques and relief positions at most. Conclusion: The midwife is an important person not only for the pregnant and giving-birth woman. Thanks to her education, experience and advice she should be able to give advice to every woman in any field concerning pregnancy, childbirth, puerperium and childcare. In the contemporary possibilities the woman has a choice, how to prepare herself for the childbirth and she can also decide, where will the chilbirth take place.
