Effect of long-term therapy in patients with familial hypercholesterolemia on the achievement of LDL-cholesterol targets, depending on the cause of the disease

Abstract:

Disertační práce se zabývá retrospektivní analýzou dat pacientů s familiární hypercholesterolemií (FH), kteří jsou dispenzarizováni a léčeni ve specializované ambulanci při Oddělení klinické biochemie Fakultní nemocnice u sv. Anny v Brně. V teoretickém úvodu je popsána problematika dyslipidémií se zaměřením na FH. Ve druhé části práce je popsán studovaný soubor pacientů s FH a jsou uvedeny výsledky analýzy získaných dat. Byla zjišťována úspěšnost dosahování cílových hodnot LDL cholesterolu, Apolipoproteinu B a Non-HDL cholesterolu a byly srovnávány hodnoty krevních lipidů ve třech podskupinách pacientů: s mutací v genu pro LDL receptor, v genu pro Apolipoprotein B a u pacientů s FH, u nichž kauzální mutace nebyla nalezena. Analýza laboratorních dat byla dále provedena v závislosti na typu léčby a na výši kardiovaskulárního rizika. Byly zjišťovány i šance na dosažení cílových hodnot krevních lipidů v závislosti na léčbě, pohlaví, prokázané mutaci a dalších identifikovaných proměnných. Výsledky práce jsou diskutovány v kontextu potřeby efektivní terapie s cílem prevence předčasných aterosklerotických kardiovaskulárních komplikací.

Abstract:

The dissertation deals with a retrospective analysis of data from patients with familial hypercholesterolemia (FH) treated in a specialized outpatient clinic at the Department of Clinical Biochemistry of St. Anne's University Hospital in Brno. The theoretical introduction describes the problems of dyslipidaemias with a focus on FH. In the second part of the dissertation, the study population of patients with FH is described and the results of the data analysis are presented. The success rate of achieving the target values of LDL cholesterol, Apolipoprotein B and Non-HDL cholesterol was investigated and blood lipid values were compared in three subgroups of patients: those with mutation in the LDL receptor gene, in the Apolipoprotein B gene and in patients with FH in whom the causative mutation was not found. The laboratory data were further analysed in groups of FH patients according to the type of treatment and cardiovascular risk category. The odds of achieving target blood lipid levels were also determined according to treatment, sex, proven mutation, and other identified variables. The results are discussed in the context of the need for effective therapy to prevent premature atherosclerotic cardiovascular events.
