Post-mortem biochemical determination in forensic practice in sudden death

Abstract:

Cíl práce: Ověření diagnostické efektivity a návrh algoritmu postmortální biochemické analýzy v rámci rozdílných biologických matric u náhlých úmrtí s předpokládanou kardiální příčinou smrti a/nebo s dysbalancí v glukózovém metabolismu. Materiál a metody: Do studie bylo zahrnuto celkem 68 zemřelých, z toho 32 tvořilo kontrolní skupinu, 36 skupinu případů (tabulka 2, příloha 6). Případová skupina zemřelých byla dále stratifikována na kardiální příčinu úmrtí, zemřelé v důsledku akutní hypoglykemické a hyperglykemické poruchy (příloha 7). U všech zemřelých byla postmortem odebrána krev, sklivec a, byla-li k dispozici, i perikardiální tekutina. V biologickém materiálu, který byl preanalyticky upraven a analyzován pomocí analytických systémů Atellica Solution (Siemens) a ABL800 Flex (Radiometer), byly stanoveny vybrané laboratorní parametry. Výsledky: Naměřená data byla následně statisticky zpracována pomocí programů MedCals (verze 18.10), Stata (verze 14), a internetovým kalkulátorem Social Science Statistics, kde byla konfrontována statistická významnost případové a kontrolní skupiny u vybraných laboratorních analýz v různých biologických matricích. Dominující efektivita stanovení vybraných parametrů byla pozorována u případové skupiny s předpokládanou poruchovou v glukózovém metabolismu. Statisticky významná korelace byla nalezena při stanovení hladin glukózy v krvi (p <0,001) a sklivci (p <0,001) mezi případovou a kontrolní skupinou (tabulky 6-9, grafy 1 a 2), u ostatních parametrů nebyla dosažena statistická významnost (tabulky 10-21, grafy 3-7). Statistické výstupy kardiálních markerů však poskytly rozporuplné závěry. V rámci stanovených parametrů (troponinu I, NT-proBNP a myoglobinu) nebyla nalezena signifikantní statistická významnost mezi kontrolní a případovou skupinou (předpokládaná kardiální příčina náhlého úmrtí) a to jak v krvi, tak ve sklivci (tabulky 25-36, grafy 10-15). Závěr: Pro potřeby postmortální biochemické analýzy bylo na základě dosažených statistických výstupů doporučeno analyzovat krev před všeobecně upřednostňovaným sklivcem. Optimální panel vyšetření k verifikaci úmrtí v rámci poruch glukózového metabolismu zahrnuje kvantifikaci glukózy, laktátu, C-peptidu a inzulinu. Použití kardiálních markerů v postmortální diagnostice k určení příčiny smrti nepřináší očekávaný benefit. Limitujícím faktorem studie byla velikost testovaného souboru a znalost podrobných anamnestických dat.

Abstract:

Aim of the work: Verification of diagnostic effectivity and proposal of a postmortem biochemical analysis algorithm within different biological matrices in sudden deaths with a presumed cardiac cause of death and/or with imbalance in glucose metabolism. Materials and methods: The study material consisted of 68 deaths, whose causes of death were categorized, 32 deaths in control group, 36 deaths in group with a specific cause of death. Group with specific cause of death was stratified into the following groups: sudden cardiac deaths, deaths with hyperglycemia and deaths with hypoglycemia complication. In all deaths were aspired postmortem blood, vitreous humor and pericardial fluid. Samples were prepared before analysis and measured on the different analytical platforms Atellica Solution (Siemens) and ABL 800 Flex (Radiometer), on which we analyzed selected parameters. Results: All statistical analyses were performed using MedCalc (version 18.10), Stata (version 14), online calculator Social Science Statistics, and comparisons were performed using the control group and group with specific cause of death in different postmortem biological materials. The predominant effectivity of the determination of selected parameters was observed in the case group with presumed impaired glucose metabolism. A statistically significant correlation was found in the determination of blood glucose levels (p <0.001) and vitreous humor (p <0.001) between the case and control groups (tables 6-9, graphs 1 and 2), no statistical significance was achieved for other parameters (tables 10 -21, graphs 3-7). However, the statistical outputs of cardiac markers provided conflicting conclusions. Within the determined parameters (troponin I, NT-proBNP and myoglobin) no significant statistical significance was found between the control and case group (presumed cardiac cause of sudden death), both in the blood and in the vitreous humor (tables 25-36, graphs 10-15). Conclusions: For the needs of postmortem biochemical analysis, it was recommended to analyze blood instead of the generally preferred vitreous humor, based on the achieved statistical results. The optimal panel of tests to verify deaths in glucose metabolism disorders includes quantification of glucose, lactate, C-peptide, and insulin. The use of cardiac markers in postmortem diagnosis to determine the cause of death does not provide the expected benefit. The limiting factor of the study was the size of the tested group and the knowledge of detailed anamnestic data.
