Prognostický význam biomarkerů u akutního koronárního syndromu

Prognostic value of biomarkers in patients with acute coronary syndrome

Abstract:

Úvod Akutní koronární syndrom (AKS) zahrnuje akutní infarkt myokardu a nestabilní anginu pectoris. Akutní infarkt myokardu je celosvětově hlavní příčinou mortality. Tato práce je rozdělena na část teoretickou, která se zabývá diagnostikou a nedůležitějšími biomarkery v oblasti akutních koronárních syndromů, a na část praktickou, která je zaměřena na prognostický význam dvou biomarkerů u pacientů s akutním koronárním syndromem. Těmito biomarkery jsou sCD146, protein, jehož uvolňování se do krevní plazmy bylo popsáno při systémovém městnání a tato práce nově popisuje diagnostický přínos u pacientů s akutním koronárním syndromem. Druhým biomarkerem je cystatin C, marker k diagnostice akutního poškození ledvin, který u pacientů s AKS vykazuje význam prognostický. Metodika V práci s sCD146 bylo vyhodnoceno celkem 927 konsekutivních pacientů s AKS. Plazmatické hodnoty sCD146, BNP (brain natriuretic peptide) a vysoce senzitivního troponinu T byly změřeny do 24 hodin od vzniku bolestí na hrudi. Dle tíže plicní kongesce na scintigramu hrudníku byli pacienti rozděleni do tří skupin podle tíže plicního městnání. V práci s cystatinem C bylo vyhodnoceno celkem 309 pacientů, bylo u nich spočítáno GRACE skóre a následně byli rozděleni do kvartilů podle hodnoty cystatinu C. Podle ROC analýzy byla následně porovnána prediktivní síla cystatinu C a GRACE skóre k predikci celkové roční mortality a ročních rehospitalizací pro akutní srdeční selhání. Dále byl srovnán cystatin C a kreatinin v predikci poškození ledvin. Výsledky Plazmatická hladina sCD146 koreluje s radiologicky prokázaným stupněm plicní kongesce a byla méně variabilní a lépe asociována s radiologickým stupněm plicní kongesce než BNP. Narozdíl od BNP nebyly hodnoty sCD146 ovlivněny hodnotou troponinu, protože s rostoucí hodnotou troponinu T klesala hodnota sCD146, což je nová informace k tomuto biomarkeru a ukazuje na extrakardiální zdroj sCD146. Samotná hodnota cystatinu C je srovnatelná v predikci roční mortality, rehospitalizací pro AHF a kombinovaného end-pointu s GRACE skóre. Kombinace GRACE a cystatinu C přinesla zpřesnění prognózy ve srovnání se samotným GRACE pouze ve skupině více rizikových pacientů s GRACE ≥ 148 bodů. Zvýšená vstupní hodnota cystatinu C ≥ 1.09 mg/l je oproti kreatininu lepším prediktorem akutního poškození ledvin. Závěr V této práci byl prokázán vztah mezi hodnotami sCD146 a závažností plicního městnání u pacientů s AKS bez ohledu na velikost ischemického ložiska myokardu. Cystatin C zpřesňuje prognostický význam GRACE skóre pro odhad roční mortality, rehospitalizací pro AHF i kombinovaného end-pointu u pacientů s AKS. Navíc stratifikuje pacienty s rizikem rozvoje akutního poškození ledvin.

Abstract:

Background Acute Coronary Syndrome (ACS) includes acute myocardial infarction and unstable angina. Acute myocardial infarction is now the main reason for mortality worldwide. This work is focused on the prognostic value of two biomarkers for patients with ACS. One of these biomarkers is sCD146, a protein whose release into blood plasma has already been described by patients with heart failure, and this work now newly describes the diagnostic benefit of sCD146 for patients suffering from ACS. The second biomarker is Cystatin C, a biomarker for the diagnosis of acute kidney damage, which has prognostic value in patients with ACS. Methodology In this work with sCD146, a total of 927 patients with ACS were evaluated consecutively. Plasma levels of sCD146, BNP (brain natriuretic peptide), and the high-sensitive troponin T were all measured within 24 hours after the onset of chest pain. Pulmonary congestion according to chest X-ray was determined and the patients were divided into three groups according to the degree of congestion. A total of 309 patients were evaluated for Cystatin C. GRACE score was counted and the patients then divided into quartiles according to their Cystatin C values. By using ROC analysis, the predictive strength of the cystatin C and GRACE scores were then compared to the prediction of total annual mortality and annual rehospitalisations for Acute Heart Failure (AHF). Cystatin C and creatinine were also compared for a prediction of acute kidney damage. Results The plasma level of sCD146 reflected the radiologically proven severity of pulmonary congestion and was less variable and better associated with the radiological degree of pulmonary congestion than with BNP. Unlike with BNP however, the sCD146 values were not affected by the troponin T value, because with an increasing value of troponin T, the sCD146 value decreases. This is new information for this biomarker sCD146 and shows at the extracardial origin of sCD146. The cystatin C predictive value is comparable with the GRACE score for all-cause annual mortality, rehospitalisations for AHF, and the combined end-point. The combination of the GRACE score and Cystatin C seems to be better just for high-risk patients with a GRACE score ≥ 148 points. The increased cystatin C ≥ 1.09 mg / l intake value is a better predictive of acute renal impairment than creatinine. Conclusion This work proves the relation between the sCD146 values and the severity of lung congestion according to X-rays in patients with ACS. There is no influence of the size of the ischemic centre to the sCD146 values. Cystatin C shows the prognostic value of the GRACE score in patients with ACS to be more accurate for annual all-cause mortality, rehospitalisations for AHF, and combined end-point. In addition it stratifies the patients with acute kidney damage.
