Coordinated Rehabilitation in the Process of Dismissing Clients with Disabilities from the Medical Facility

Abstract:

Předkládaná diplomová práce je zaměřená na patologické stárnutí, potřeby seniorů s disabilitou a koordinovanou rehabilitaci jako možnost naplňování potřeb a autonomie potřebných seniorů. Cílem diplomové práce je zjistit, jak probíhá proces propouštění u seniorů s geriatrickými syndromy ze zdravotnického zařízení, jak jsou mapovány jejich potřeby v návaznosti na propuštění a koordinaci pomoci. Diplomová práce obsahuje teoretickou a výzkumnou část. Teoretická část je rozdělena do čtyř kapitol, kde jsou popsány důležité pojmy k pochopení dané problematiky. Výzkumná část diplomové práce je zpracovaná kvalitativním výzkumem, metodou výzkumu byla zvolena případová studie. Hlavním cílem výzkumu je zjistit, jak probíhá proces propouštění seniorů s geriatrickými syndromy ze zdravotnického zařízení a jak jsou naplňovány jejich potřeby a autonomie.

Abstract:

This diploma thesis is focused on pathological aging, needs of the elderly with a disability and coordinated rehabilitation as a possibility of covering their needs and autonomy. The goal of the thesis is to scrutinize the process of discharging aging ones with geriatrics syndromes from medical facilities and the way of mapping their needs following the discharge and coordination of subsequent help. The diploma thesis consist of a theoretical section and a research section. The theoretical section is divided into four chapters where the main terms are described to make the given issues understood. The research section undertakes the qualitative research with a case study as the chosen method. The main goal of the research is to discover the process of discharging aging ones with geriatrics syndromes from medical facilities and the ways their needs and autonomy are being full-filled.
