Phakic intraocular lens

Abstract:

Fakické nitrooční čočky představují jednou z možností korekce vysokých forem ametropie. Fakické čočky se začali implantovat v polovině sedumdesátych let, ale kvůli četnosti komplikací, které spůsobovali se od jejich užití upustilo. Renesanci pak zažili v polovině osumdesátych let díky inovovanému designu. Rozdelujeme je na předněkomorové a zadněkomorové. Předněkomorové lze fixovat na duhovku nebo je lze fixovat do rohovko-duhovkového úhlu. Zadněkomorové se nejčastěji implantují do sulcus ciliaris. Komplikace spojené s předněkomorovými fakickými čočkami jsou sekundární glaukom, pokles počtu endotelových buněk, ovalizace zornice, nestabilita a rotace fakické čočky. U zadněkomorových čoček se kromě poklesu počtu endotelových buněk může vyskytnout komplikovaná katarakta a disperze pigmentu duhovky. Pacienti indikovaný k implantaci fakických čoček musí mít počet endotelových buněk vyšší než 2500/mm2 a hloubku přední komory větší nez 3,2 mm. Fakické nitrooční čočky lze také kombinovat s laserovými operačními postupmi. Táto technika se jmenuje Bioptics.

Abstract:

Phakic intraocular lens represents one of possibility for correction of high ametropia. Surgeons started implant the phakic intraocular lens in the middle of 70´s but because of lots of complications that occur the surgeons break away from this procedure. In the middle of 80´s the surgeons started implant them again due to new and better desing. According to the position that they replace they can be anterior chamber and posterior chamber. The anterior chamber lens are classified into angle supported and iris supported. The posterior chamber lens are mainly implanted into the sulcus ciliaris. Complications that the anterior chamber lens can induce are secondary glaucoma, decrease of the number of endothelial cells, pupil ovalization, rotation and displacement of the phakic lens. The posterior chamber lens can induce decrease of the number of endothelial cells and also complicated cataract and iris pigment dispersion. Patients who are indicated for implatation of phakic intraocular lens must have the count of endothelial cells more than 2500/mm2 and depth of the anterior chamber more than 3,2 mm. It is also possible to combine the phakic intraocular lens with the laser operation. This technique calls Bioptics.
